The clinical picture of acute cholecystitis complicated by jaundice has four main types: with the prevailing syndrome of inflammation of the gallbladder, of the pancreatic syndrome, of the cholangitis syndrome and of the profound pain syndrome by the type of renal colic. Echography is thought to be an informative preoperative method of diagnostics of the cause of jaundice, its significance being increased when combined with the percutaneous transhepatic cholangiography (94.4%).

The authors suggest an original optic attachment to a microsurgical forceps which removes the shortcomings of the traditional optic apparatuses used in microsurgery. The attachment allows the lens to be brought directly to the operative zone and combines the functions of two instruments--forceps and magnifying glass. Such a combination expands considerably the possibilities of the operator and creates optimal conditions for precision manipulations.

Based on literature and personal data (146 choledochotomies) the authors have developed a scheme of contraindications, indications and conditions for using the precision suture of the choledochus in patients with benign lesions of bile ducts. For this method of completing choledochotomy to be realized, the indications should be correlated with the complex of conditions. To most important conditions the authors refer the possibility to use the precision technique as well as the delicate highly informative diagnostic and curative endocholedochal manipulations.
