A total synthesis of the enantiopure syn,syn-tosyl-samroiyotmycin A, a C-symmetric 20-membered antimalarial macrodiolide with syn,syn-configuration of the 8,24-dihydroxy-9,25-dimethyl units and it's enantiopure anti,anti-derivative is described. The synthesis was accomplished utilizing a linear approach in 7 steps and 3 % overall yield via a sequence of diastereoselective methylation of SuperQuat oxazolidinone auxiliary, cross metathesis and Yamaguchi macrolactonization of fully functionalized seco-acids. By a similar approach we gained access to several samroiyotmycin analogues and precursors.

Objective: Traditional pain management pathways following craniotomy are predicated on opioids. However, narcotics can confound critical neurological examination, contribute to respiratory depression, lower the seizure threshold, and lead to medication habituation, dependence, and/or abuse. Alternative medications to better address postoperative pain while mitigating opioid-related adverse effects remain insufficiently studied.

Article Synopsis
  • Lipid nanoparticles (LNPs) are currently the best method for delivering messenger RNA (mRNA), and a new, cost-effective microfluidic setup has been developed for their synthesis.
  • This new method allows for precise control of LNP characteristics and shows better structural uniformity compared to LNPs made with commercial equipment or ethanol injection.
  • While the manufacturing process impacts the LNP's structure, it has only a minor effect on how effectively mRNA is delivered both in lab settings and in living organisms.

We present a case of a 55-year-old woman presenting for an elective minor procedure. Following a previous anaesthetic, a 'can't intubate, can't oxygenate' incident had been documented. She had since undergone awake tracheal intubation for procedures requiring general anaesthesia.
