Outpatient parenteral antimicrobial therapy with continuous infusion of meropenem: A retrospective analysis of three years of clinical experience.

Introduction: Data regarding outpatient parenteral antimicrobial therapy (OPAT) with continuous infusion of meropenem (CIM) remain scarce and controversial. We aimed to analyze its outcomes.

Methods: We conducted a retrospective analysis of a cohort of patients who received OPAT with CIM during a three-year period at a single center in northwest Spain.

Evaluation of a Telemedicine pharmaceutical care program for coordination patient's transfer between hospital and primary care, using Telepharmacy.

Objective: To report our experience with Telemedicine projects: a Telepharmacy Hospital Pharmacy/Primary Care Pharmacy Coordination Program and a Hospital Pharmacy/Primary Care Pharmacy  Electronic Cross-consultation Program. Results are reported in terms of  medication adherence, perceived quality and satisfaction, and economic  impact.

Method: A) Telepharmacy Hospital Pharmacy/Primary Care Pharmacy Coordination Program: Phases of development: 1) Creation of a  work group; 2) definition of patient inclusion criteria; 3) selection of medicines; 4) integration of hospital and primary care pharmaceutical care; 5)  setting up of facilities in primary care; 6) logistics design; 7) creation of the Telemedicine system; 8) provision of training to primary care pharmacists; 9) establishment of a pharmaceutical care protocol; 10) obtaining patient informed consent.

Continuous infusion of antibiotics using elastomeric pumps in the hospital at home setting.

Objective: To describe the avoided costs and to analyze the effectiveness of intravenous antibiotic treatment in continuous perfusion in patients at Hospital at Home Units (HHU) administered using elastomeric infusion pumps (EIP) prepared in a Hospital Pharmacy Service (HPS).

Methods: Retrospective observational study of the number and type of EIP prepared in the HPS and of the treated patients. Study period: January 2017-December2018.

[Quality indicators in the storage and dispensing process in a Hospital Pharmacy].

Objective: To establish indicators for the evaluation of the quality of the storage and dispensing processes related to semiautomatic vertical (SAVCS) and horizontal (SAHCS) carousel systems.

Material And Methods: Descriptive observational study conducted between January-December 2012. Definition of quality indicators, a target value is established and an obtained value is calculated for 2012.
