Stereotactic body radiation therapy (SBRT) to treat spinal metastases has shown excellent clinical outcomes for local control. High dose gradients wrapping around spinal cord make this treatment technically challenging. Although wide bore computed tomography (CT) scanners provide increased space for patients, the scan field of view (sFOV) remains considerably smaller than the bore size. Consequently, patient anatomy which spans beyond the sFOV is truncated and the information is lost. As a solution, some manufacturers provide the capacity to reconstruct CT images from a partial dataset at an extended field of view (eFOV).

Radiation is delivered using remote afterloaders to drive highly active sources (commonly (192)Ir with an air KERMA strength range between 20,000 and 40,000 U, where 1 U = 1 μGy m(2)/h in air) through applicators directly into the patient's prescribed region of treatment. Due to the obvious ramifications of incorrect treatment while using such an active source, it is essential that there are methods for quality assurance (QA) that can directly and accurately verify the treatment plan and the functionality of the remote afterloader.
