Three collegiate athletes tested positive for morphine after a championship competition; however, all three denied narcotic use. The medical team was stumped until the coach suggested a food source. Reviewing the literature and recreating the scenario helped identify the cause.

Stability of buffered lidocaine and epinephrine used for local anesthesia.

Buffered lidocaine has been recently recommended for local anesthesia, as there is less pain on injection of the buffered solution. Reduced pain on injection of lidocaine and epinephrine buffered to a neutral pH was confirmed in 20 subjects (P less than .01).

Comparison of aminoglycoside concentrations measured in plasma versus serum.

The terms "serum concentration" and "plasma concentration" are often used interchangeably in therapeutic drug monitoring, but few studies have addressed the comparability of serum and plasma drug concentrations. Before implementing a change in the procedure for measuring aminoglycoside concentrations in our institution, we prospectively compared values for tobramycin and gentamicin concentrations measured in serum and plasma. For the 208 samples that were tested, plasma aminoglycoside concentrations were significantly lower than those in serum.
