Background: Continuous kidney replacement therapy (CKRT) has recently become the preferred kidney replacement modality for children with acute kidney injury (AKI). We hypothesise that CKRT technical parameters and treatment settings in addition to the clinical characteristics of patients may influence the circuit lifetime in children.

Methods: The study involved children included in the EurAKId registry (NCT02960867), who underwent CKRT treatment.

Introduction: Unlike idiopathic nephrotic syndrome (NS), hereditary podocytopathies are not expected to recur after kidney transplantation. However, some reports of posttransplant recurrence of NS in patients carrying variants in the gene have been described, notably with the p.Arg138Gln variant, which is more prevalent in Europe.

Article Synopsis
  • The study focuses on urinary tract infections (UTIs) in children and investigates the potential use of soluble Toll-like receptors (sTLR4, sTLR5) and interleukin 8 (IL-8) as biomarkers for diagnosing UTIs.
  • It involved 520 children, including those with UTIs, non-UTI infections, and healthy controls, measuring urine and serum levels of the biomarkers before and after treatment.
  • Results show that urine sTLR4 levels are higher in UTI patients, with a cut-off level identified to predict UTIs, especially higher in cases of pyelonephritis compared to cystitis, and decreasing post-treatment.

Background: Alport syndrome (AS) is characterized by progressive kidney disease. There is increasing evidence that renin-angiotensin-aldosterone system (RAAS) inhibition delays chronic kidney disease (CKD) while the effectiveness of immunosuppressive (IS) therapy in AS is still uncertain. In this study, we aimed to analyze the outcomes of pediatric patients with X-linked AS (XLAS) who received RAAS inhibitors and IS therapy.

Article Synopsis
  • The study examined how the characteristics of hospitals and dialysis units affect hospitalizations in pediatric dialysis patients, specifically for access-related complications.
  • It found that children's hospitals (CHs) had more specialized staff and experience, leading to a higher use of automated peritoneal dialysis (APD) and hemodiafiltration (HDF) compared to general hospitals (GHs).
  • CAPD patients faced a higher hospitalization risk due to infections, but centers with more PD experience saw lower hospitalization rates for these patients, while conventional hemodialysis (cHD) increased the risk for non-infectious complications.

Article Synopsis
  • A new study looked at a special urine test called uHSP70 to help tell if kids have a urinary tract infection (UTI) instead of just using regular tests that might not be very reliable.
  • The study involved 802 kids and found that uHSP70 levels were much higher in kids with UTI compared to those with other infections or who were healthy.
  • Using uHSP70 could help doctors correctly diagnose UTIs and might stop about 80% of kids from getting unnecessary antibiotics.

Article Synopsis
  • - The study examines the perceptions of patients, caregivers, nurses, and physicians regarding the automated peritoneal dialysis system (Homechoice with Claria sharesource system - CSS), focusing on its advantages and disadvantages for home treatment monitoring.
  • - Questionnaires were filled out by 30 patients/caregivers, 22 pediatric nephrologists, and 15 PD nurses, revealing that most healthcare professionals believe CSS enhances patient monitoring and saves time, although many nurses felt other doctors weren't familiar with it.
  • - While 90% of patients reported feeling safer due to constant monitoring, only 20% noticed reduced hospital visits, highlighting the need for more extensive research to fully understand CSS's impact on patient quality of life.

Article Synopsis
  • The study analyzed 25 genetically confirmed cases of PLCε1-related kidney disease to explore the clinical spectrum, genotype-phenotype correlations, and treatment impacts on kidney outcomes.
  • 36% of patients presented with isolated proteinuria, while others had nephrotic syndrome or chronic kidney disease, with significant variations in histopathology results among those biopsied.
  • The findings suggest that while genetic variations do not correlate with clinical presentations, the type of kidney condition and histological features significantly influence prognosis and treatment responses.

Background: Acute kidney injury (AKI), particularly that requiring dialysis, is a severe complication in hospitalized children that is associated with high morbidity and mortality. A prospective European AKI registry (EurAKId registry, NCT02960867) was created to describe the epidemiology and outcomes of paediatric patients treated with acute dialysis.

Methods: Children were recruited who were between 0 and 18 years of age and were treated both in and outside the paediatric intensive care unit (PICU) with peritoneal dialysis (PD), haemodialysis (HD) or continuous kidney replacement therapy (CKRT) for AKI or metabolic derangement, fluid overload (FO), sepsis or respiratory distress.

Although the number of studies evaluating methods to predict fluid overload is increasing, the assessment of fluid status in children on dialysis is still fraught with inaccuracies. We aimed to evaluate the predictive capability of lung ultrasounds and the inferior vena cava collapsibility index (cIVC) in predialysis overhydration in children with end-stage kidney disease. Ten children with end-stage kidney disease who were on an intermittent hemodialysis program were included.
