One month after initiating the neoadjuvant imatinib therapy, the patient was admitted to our hospital for fever and was diagnosed with an intratumoral infection. Antibiotic therapy was immediately initiated, and upper gastrointestinal endoscopy revealed a fistula between the stomach and tumor; therefore, endoscopic drainage was performed through the fistula.

However, age and comorbidities are considered in decision-making and some surgeons opt for observation. A 58-year-old man was treated with radical excision for rectal cancer. Pathological findings were pT3N1M0, Stage ⅢB, RAS: mutant, BRAF: mutant, MSS. The patient was followed up without adjuvant chemotherapy.
