Background: Japan currently has a super-aged society, with a rapid increase in elderly patients in need of medical care. Determining treatment strategies for acute cholecystitis (AC) in very elderly patients with various comorbidities is often difficult. Although percutaneous cholecystostomy (PC) is a less-invasive treatment option, its impact on subsequent surgery remains debatable.

The awn is a bristle-like appendage that protrudes from the seed tip and plays a critical role in preventing feed damage and spreading habitats in many grass species, including rice. While all wild species in the genus have awns, this trait has been eliminated in domesticated species due to its obstructive nature to agricultural processes. To date, several genes involved in awn development have been identified in wild rice, and which are ancestral species of cultivated rice in Asia and Africa, respectively.

Background: Several hypotheses regarding the pathomechanisms of schizophrenia have been proposed. If schizophrenia is a unitary disease, then these pathological processes must be linked; however, if such links do not exist, schizophrenia may best be considered a group of disorders. Only a few studies have examined the relationships among these pathomechanisms.

An 18-step synthesis of (-)-enigmazole A is herein disclosed. The present synthesis is based on a modular assembly of three building blocks of similar complexity, a macrocyclic ring-closing metathesis to forge the 18-membered macrocyclic skeleton, and a desilylative transannular oxa-Michael addition for stereoselective construction of the 2,6--substituted tetrahydropyran ring.

  • - Granulocyte colony-stimulating factor (G-CSF) commonly causes side effects like bone pain, headache, and fatigue, but it can also lead to serious conditions like aortitis.
  • - Two cases of aortitis induced by pegfilgrastim (PEG-G), a form of G-CSF, were reported: one involving a woman with breast cancer and another with a woman post-surgery, both showing symptoms like fever and abdominal pain.
  • - Both patients were successfully treated with prednisolone after diagnosis via CT scans, which highlighted the importance of considering G-CSF as a potential cause for aortitis when fever arises post-treatment.

  • Edwardsiella tarda (E. tarda) is a rare Gram-negative bacterium that can cause severe infections like sepsis, especially in vulnerable populations like the elderly or those with certain pre-existing conditions.
  • A 65-year-old woman with fever and abdominal pain was diagnosed with cholecystitis after blood tests and imaging revealed inflammation, and cultures confirmed E. tarda infection.
  • This case highlights the unusual occurrence of acute cholecystitis due to E. tarda, where successful treatment involved surgery and targeted antibiotic therapy, leading to the patient's recovery.

Background: Growing evidence suggests that intervention for smoking cessation enhances alcohol abstinence in treatment settings for alcohol dependence. However, research in this field is rare in Asians.

Method: We prospectively investigated the association of smoking status with drinking status using 9 surveys mailed during a 12-month period in 198 Japanese alcohol-dependent men (70 never/ex-smokers and 128 smokers) who admitted for the first time and completed a 3-month inpatient program for simultaneous alcohol abstinence and smoking cessation.

Article Synopsis
  • An 80-year-old man with Parkinson's disease developed a fever and underwent a CT scan, revealing a large mass in the colon suspected to invade the duodenum, along with enlarged lymph nodes.
  • A colonoscopy confirmed the presence of a tumor, which was identified as adenocarcinoma, leading to a right hemicolectomy to remove it along with affected lymph nodes.
  • The patient's recovery was complicated by his age, requiring a prolonged hospital stay, but he has now returned home one year post-surgery without any signs of cancer recurrence, indicating that aggressive treatment can still be beneficial for elderly patients.

Two species of rice have been independently domesticated from different ancestral wild species in Asia and Africa. Comparison of mutations that underlie phenotypic and physiological alterations associated with domestication traits in these species gives insights into the domestication history of rice in both regions. Asian cultivated rice, and African cultivated rice, , have been modified and improved for common traits beneficial for humans, including erect plant architecture, nonshattering seeds, nonpigmented pericarp, and lack of awns.

Although immune checkpoint inhibitors (ICIs) can be used for lung cancer treatment, the activated immune response may cause immune-related adverse effects (irAEs). We present here a case of cytomegalovirus (CMV) enterocolitis during steroid therapy for an irAE. A 70-year-old man diagnosed with small-cell lung carcinoma (limited disease) received radiotherapy plus two chemotherapy cycles of cisplatin and etoposide.

Background: While several studies have revealed that neurodevelopmental disorders have a high probability of overlapping with substance use disorders, the effects of neurodevelopmental disorders on the courses of substance use disorders have hardly been examined.

Methods: This study targeted 637 alcohol-dependent individuals who received inpatient treatment and whose drinking situations were followed for 12 months after hospital discharge using mailed questionnaires. The comorbidity of psychiatric disorders and the characteristics associated with the neurodevelopmental disorders were assessed using several measurements at the time of hospital admission.

Background: Foreign body ingestion is a common case in daily medical care, and it usually passes through the entire gastrointestinal tract naturally and is excreted in the feces. However, long and sharp foreign bodies may be difficult to pass naturally due to their shape. Here, we present a rare case of a duodenal foreign body, a toothbrush, that required laparoscopic surgical removal after a failed endoscopic attempt.

Article Synopsis
  • The study investigates how the severity of alcohol dependence affects recovery outcomes following inpatient treatment, with a focus on drinking behavior over a 12-month period.
  • Results indicate that patients with higher severity of dependence, as measured by diagnostic criteria and Alcohol Dependence Scale scores, had lower abstinence rates and increased likelihood of relapse.
  • The findings suggest that greater severity of alcohol dependence is linked to less favorable drinking patterns and overall recovery post-treatment.
