Background: Implantable cardiac monitors (ICMs) provide valuable insights into managing atrial fibrillation (AF). Data suggest that ICMs increase AF detection, but their impact on management is still uncertain.

Objectives: To evaluate and compare the impact of ICMs on the clinical management of AF.

Aim: To evaluate the ability of blood-biomarkers, clinical examination, electrophysiology, or neuroimaging, assessed within 14 days from return of circulation to predict good neurological outcome in children following out- or in-hospital cardiac arrest.

Methods: Medline, EMBASE and Cochrane Trials databases were searched (2010-2023). Sensitivity and false positive rates (FPR) for good neurological outcome (defined as either 'no, mild, moderate disability or minimal change from baseline') in paediatric survivors were calculated for each predictor.

Implementation of behavior therapy for ADHD faces challenges in community settings. We describe development of a community-based implementation model for adolescent ADHD behavior therapy (Supporting Teens' Autonomy Daily; STAND) blended with Motivational Interviewing (MI). A stakeholder-engaged development approach is used based on the Exploration, Preparation, Implementation, Sustainment (EPIS) framework.

Background: US chiefs of police hold significant influence over the perceived acceptability and appropriateness of interventions for opioid use disorder (OUD) among the public, elected officials, and subordinate officers. This study assessed whether police chiefs' support for such interventions was sensitive to framing an intervention's benefits in terms that emphasize public health and harm reduction outcomes, versus terms typically indicative of public safety outcomes.

Methods: A two-armed survey utilizing a randomized, between-subjects design tested framing-based variance in support among US chiefs of police for overdose prevention centers, syringe service programs (SSPs), Good Samaritan laws, police naloxone distribution, trustworthiness of officers in recovery from OUD, and related propositions.

This is the eighth annual summary of the International Liaison Committee on Resuscitation International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations; a more comprehensive review was done in 2020. This latest summary addresses the most recent published resuscitation evidence reviewed by the International Liaison Committee on Resuscitation task force science experts. Members from 6 International Liaison Committee on Resuscitation task forces have assessed, discussed, and debated the quality of the evidence, using Grading of Recommendations Assessment, Development, and Evaluation criteria, and their statements include consensus treatment recommendations.

Introduction: Endoscopy, standard-of-care for monitoring eosinophilic esophagitis (EoE), assesses mucosal inflammation. The Esophageal String Test (EST), a minimally invasive swallowed capsule and immunoassays, quantifies EoE inflammation. We determined whether the EST/EoEScore can monitor disease in patients undergoing treatment.

  • Left atrial appendage closure (LAAC) is used as an alternative stroke prevention method for patients who cannot take blood thinners, but pulmonary artery injury (PAI) is a rare and serious complication associated with it.
  • A review of previous PAI cases and data from the FDA's MAUDE database identified 36 cases, predominantly involving older patients, and highlighted that PAI often presents soon after the procedure, with a significant percentage requiring surgery.
  • The study emphasizes the need for heightened awareness of PAI symptoms post-LAAC, especially concerning rapid fluid accumulation around the heart, even without clear predictive signs from imaging or clinical details.

  • Problem-solving courts (PSCs) aim to provide alternatives to traditional prosecution and support for individuals with drug-related issues, particularly those facing challenges with opioid use disorder, but medications like methadone and buprenorphine remain underutilized among PSC clients.
  • A study involving semi-structured interviews with 54 PSC staff across four states revealed that most courts have lifted blanket prohibitions on medication for opioid use disorder (MOUD) due to changes in policies and education, but staff preferences and concerns still influence individual client decisions.
  • The decision-making factors for allowing MOUD include the type of treatment provider, communication with the court, and adherence to certain criteria, with staff expressing more comfort towards buprenorphine than methadone

Background: The use of cardiopulmonary resuscitation (CPR) and automated external defibrillators (AEDs) significantly improve the chances of survival after out-of-hospital cardiac arrest. Subsequently, state laws mandate training in CPR and AED use for high school graduation. However, training and its impact vary and must be better understood.

Background: A 6-food elimination diet in pediatric eosinophilic esophagitis (EoE) is difficult to implement and may negatively affect quality of life (QoL). Less restrictive elimination diets may balance QoL and efficacy.

Objective: We performed a multisite, randomized comparative efficacy trial of a 1-food (milk) elimination diet (1FED) versus 4-food (milk, egg, wheat, soy) elimination diet (4FED) in pediatric EoE.

Natural language processing (NLP) is a subfield of machine learning that may facilitate the evaluation of therapist-client interactions and provide feedback to therapists on client outcomes on a large scale. However, there have been limited studies applying NLP models to client outcome prediction that have (a) used transcripts of therapist-client interactions as direct predictors of client symptom improvement, (b) accounted for contextual linguistic complexities, and (c) used best practices in classical training and test splits in model development. Using 2,630 session recordings from 795 clients and 56 therapists, we developed NLP models that directly predicted client symptoms of a given session based on session recordings of the previous session (Spearman's rho =0.

Introduction: Hemostasis following large-bore femoral vein access remains a challenge. Manual compression has been the standard of care but requires bedside staff, prolonged bed rest, and longer length of stay. The LockeT is an external compression device that attempts to address these issues while achieving venous hemostasis.

  • Multiple states have implemented CPR training for high school students and staff to reduce sudden cardiac death (SCD), but the quality and execution of these mandates differ significantly across the country.
  • A scoring system was created to objectively assess CPR training mandates in all 50 states and the District of Columbia, revealing a wide range in scores that indicate the robustness of these mandates.
  • The study's findings suggest that although there is variability in state mandates, no strong link was found between CPR training requirements and reduced SCD rates, underscoring the need for more consistent and effective policies.

  • - Atrial fibrillation (AF) is a common heart rhythm disorder, and the way it's treated in emergency rooms varies widely, affecting patient care and access to effective treatments.
  • - The study, which involved 500 patients, compared outcomes between those treated through an organized pathway for AF and those receiving routine care, finding significant advantages for the organized approach.
  • - Results showed that patients on the organized pathway had faster access to treatments like ablation and anticoagulation, leading to shorter hospital stays and better overall outcomes compared to the control group.

Objectives: To assess the prevalence of emergency medical incidents wherein naloxone was administered but overdose was not described as the chief complaint during the 9-1-1 call, including differences by overdose victim race/ethnicity and sex.

Methods: We computed the percentage of 9-1-1 calls in Marion County, Indiana, from 2011 to 2020, wherein naloxone was administered but the caller did not describe overdose as the chief complaint. We estimated a logistic regression to examine the associations between reporting of overdose as the chief complaint and race and sex of the overdose victim.

Background: Annually 15,200 children suffer an in-hospital cardiac arrest (IHCA) in the US. Ventricular fibrillation or pulseless ventricular tachycardia (VF/pVT) is the initial rhythm in 10-15% of these arrests. We sought to evaluate the association of number of shocks and early dose escalation with survival for initial VF/pVT in pediatric IHCA.
