Diagnostic accuracy of low dose CT-Scan abdomen in patients with clinical features of acute appendicitis.

Background & Objective: Acute appendicitis is one of the commonest causes of acute abdominal pain presenting to emergency department (ED) and Computerized Tomography scan (CT) is considered gold standard for its diagnosis. Internationally Low Dose Computerized Tomography scan (LDCT) in emergency department is recommended as a beneficial tool to diagnose acute appendicitis with less exposure to radiation and reduction in the rate of negative laparotomy. Local trials are needed to determine the diagnostic accuracy of LDCT as the first line imaging test for acute appendicitis.

Dysregulation of Gamma Delta T-Cells, CD8+ T Cells, CD4+ T Cells, and Natural Killer Cells in Brain Pathology: Understanding HIV-Induced Impairment and Exploring Targeted GSH Therapy.

This review comprehensively explores the dysregulation of Gamma Delta T-cells, CD8+ T Cells, and Natural Killer T Cells in the context of Human Immunodeficiency Virus (HIV) infection and its implications for brain pathology. It encompasses an overview of the HIV disease process, immune cell dysregulation, association with neurological diseases, and the critical role of Glutathione (GSH) in T-cell function. The alterations in Gamma Delta T-cells during chronic infection, the intricate dynamics of Vδ1 and Vδ2 subsets, and the potential of Vγ9Vδ2 T cells in inhibiting HIV replication are discussed.

Improvement in variance estimation using transformed auxiliary variable under simple random sampling.

This paper offers a novel approach to formulate efficient ratio estimator of the population variance using a transformed auxiliary variable. The impact of transformation on auxiliary information has also been discussed. It is observed that incorporating a transformed auxiliary variable result in a high gain in efficiency.

Metastasis of lung adenocarcinoma to the pituitary gland.

Pituitary metastasis (PM) is rare occurrence and accounts for <1% of all intracranial metastatic lesions. In this study, we highlight the importance of considering atypical sites for lung adenocarcinoma metastasis by presenting a case of a 54-year-old male who was diagnosed with PM 15 months after being diagnosed with stage IV lung adenocarcinoma with metastasis to the spine, pelvis, left frontal lobe, and right occipital lobe. He was on a prolonged course of chemotherapy during those 15 months and received palliative radiation for his brain metastasis with subsequent remission after 5 months following his initial diagnosis.

Use of diesel and emulsified diesel in CI engine: A comparative analysis of engine characteristics.

Despite a number of efforts to evaluate the utility of water-diesel emulsions (WED) in CI engine to improve its performance and reduce its emissions in search of alternative fuels to combat the higher prices and depleting resources of fossil fuels, no consistent results are available. Additionally, the noise emissions in the case of WED are not thoroughly discussed which motivated this research to analyze the performance and emission characteristics of WED. Brake thermal efficiency (BTE) and brake specific fuel consumption (BSFC) were calculated at 1600 rpm within 15%-75% of the load range.

A Gull Alpha Power Weibull distribution with applications to real and simulated data.

In this paper, we produced a new family of distribution called Gull Alpha Power Family of distributions (GAPF). A Special case of GAPF is derived by considering the Weibull distribution as a baseline distribution called Gull Alpha Power Weibull distribution (GAPW). The suitability of the proposed distribution derives from its ability to model both the monotonic and non-monotonic hazard rate functions which are a common practice in survival analysis and reliability engineering.

Lomax exponential distribution with an application to real-life data.

In this paper, a new modification of the Lomax distribution is considered named as Lomax exponential distribution (LE). The proposed distribution is quite flexible in modeling the lifetime data with both decreasing and increasing shapes (non-monotonic). We derive the explicit expressions for the incomplete moments, quantile function, the density function for the order statistics etc.

Introduction: There is variation in margin policy for breast conserving therapy (BCT) in the UK and Ireland. In response to the Society of Surgical Oncology and American Society for Radiation Oncology (SSO-ASTRO) margin consensus ('no ink on tumour' for invasive and 2 mm for ductal carcinoma in situ [DCIS]) and the Association of Breast Surgery (ABS) consensus (1 mm for invasive and DCIS), we report on current margin practice and unit infrastructure in the UK and Ireland and describe how these factors impact on re-excision rates.

Do we need a different organ allocation system for kidney transplants using donors after circulatory death?

Background: There is no national policy for allocation of kidneys from Donation after circulatory death (DCD) donors in the UK. Allocation is geographical and based on individual/regional centre policies. We have evaluated the short term outcomes of paired kidneys from DCD donors subject to this allocation policy.

Outcome and cost analysis of induction immunosuppression with IL2Mab or ATG in DCD kidney transplants.

Background: Kidney transplantation from DCD now represents a significant part of the overall transplant activity in the UK. Dengue virus (DENV) and tick-borne encephalitis virus (TBEV) are flaviviruses, which can cause lethal hemorrhagic fever and encephalitis, respectively. Here, we demonstrate that the TBEV-NS5 and DENV-NS5 proteins use an internal binding mechanism to target human PDZ proteins. TBEV-NS5 has high affinity to regulating synaptic membrane exocytosis-2 (RIMS2) and Scribble, whereas DENV-NS5 binds primarily to the tight junction protein zonula occludens-1 (ZO-1).

Infective endocarditis in patients with congenitally malformed hearts: characterization of the syndrome in a developing country.

Objective: Cardiac surgery for correction or palliation of congenital cardiac disease in infancy and childhood remains a privilege that is rarely accessible to two-thirds of the world's population. This imbalance has created a unique spectrum of illness in patients with underlying congenital cardiac disease and complicating infective endocarditis in developing countries, including Pakistan. In this study, we characterize endocarditis as seen in such patients presenting in Karachi.
