Introduction: The MERIT study in Malaysia is a real-world retrospective, observational, multicenter study that evaluated asthma control in patients with uncontrolled asthma who were switched from as-needed (pro re nata [PRN]) budesonide/formoterol or inhaled corticosteroid (ICS) whenever a short-acting beta-agonist (SABA) was taken, to proactive regular dosing of fluticasone propionate/salmeterol (FP/SAL PRD).

Methods: Data from the medical records of patients who were stepped up to FP/SAL PRD were extracted retrospectively at baseline and follow-up (between 3 and 6 months after stepping up to FP/SAL PRD). The primary endpoint was the percentage of patients with improvement in asthma control assessed via the Asthma Control Test (ACT).

Tracheal stenosis is a common complication of endotracheal intubation or tracheostomy, resulting in significant morbidity and mortality. Bronchoscope interventions have been proposed as a safe alternative for the management of post-intubation post-intubation tracheal stenosis (PITS). Data for patients diagnosed with PITS across two hospitals, between 2021 and 2022, encompassing demographic, clinical, and procedural details were gathered from electronic medical records, and analysed.

Article Synopsis
  • This study examined the effectiveness and safety of lower doses of osimertinib (20 mg and 40 mg daily) for patients with advanced NSCLC who had previously failed other treatments, compared to the standard 80 mg daily dose.
  • Researchers analyzed data from 22 patients who had the EGFR T790M mutation and found that lower doses resulted in a notable overall response rate of 77.3%, with median progression-free survival of 10 months and overall survival of 13 months.
  • The findings suggest that even patients with poor performance status (ECOG 3 or 4) experienced decent survival outcomes, emphasizing that lower doses of osimertinib could be a viable treatment

Objective: Inappropriate use of short-acting beta2-agonists (SABA) in asthma has been associated with undesired outcomes. This national expert consensus was developed to increase awareness of SABA overuse and provide recommendations on the ways to eliminate SABA overprescription and overreliance in Malaysia.

Data Sources: This expert consensus was developed by searching the PubMed database, using index terms to identify SABA overuse-related burden and recommendations made in asthma guidelines.

There is limited data on the use of benralizumab in patients with severe asthma, who have allergic bronchopulmonary aspergillosis (ABPA). We report the case of a 65-year-old woman with combined severe eosinophilic and allergic asthma, who presented with refractory respiratory symptoms, hypereosinophilia and high immunoglobulin E (IgE) level. The patient had consistently poor Asthma Control Test (ACT) scores, despite a maximum dose of inhalation therapy.

Article Synopsis
  • Relapsing polychondritis (RP) is a rare multisystem disease that can severely affect the airway, potentially leading to fatal consequences for nearly half of those diagnosed.
  • Treatment options include immunotherapies and endobronchial stenting; however, stenting comes with its own risks and can increase overall complications.
  • A case study involving a 76-year-old man demonstrated that placing multiple stents in his left main bronchus significantly improved his breathing and daily activities, highlighting the importance of timely interventions for RP-related airway issues.
