Introduction: Fixed drug eruptions (FDEs) are cutaneous hypersensitivity reactions due to an offending drug and are rarely associated with nail involvement. Moreover, FDEs associated with fluconazole use are sparsely reported in literature.

Case Presentation: We report a case of a recurrent FDE with involvement of one finger and corresponding Beau's lines due to fluconazole use.

A 3 year-old boy presented to our clinic with a 3-day history of itchy rash involving the face and limbs and recent contact with a caterpillar. A diagnosis of caterpillar dermatitis was suspected. This condition has rarely been reported and may be underestimated in the pediatric population who may suffer more systemic symptoms associated with caterpillar contact.
