Article Synopsis
  • Spontaneous migration of retained bullets is uncommon, but it can lead to them being found far from the original wound site, as seen in a case involving a 17-year-old female.
  • After being shot in the right temporal region 12 years prior, she experienced discomfort in her neck, leading to the discovery of the bullet near the left pyriform fossa through laryngoscopy and imaging.
  • The bullet was successfully removed using a transoral CO2 laser microsurgery technique, demonstrating that bullets can unexpectedly migrate and how they can be treated effectively.

Papillary thyroid cancer usually presents with limited loco-regional disease. Rarely extrathyroidal extension to the surrounding vital structures is seen. It is rarer for metastatic node from papillary thyroid cancer to directly infiltrate into the larynx or pharynx.

Alveolar soft part sarcoma (ASPS) is a rare malignancy of head and neck region; orbit and tongue being most common subsites affected by this rare tumor. A 5-year-old female presented with computed tomography scan evidence of right-sided prestyloid parapharyngeal mass. Complete excision was performed through trans-cervical approach.

Knowledge of the variations of the pyramidal lobe of the thyroid is important for surgeons to perform complete resection of the functional thyroid tissue. Complete excision of the thyroid tissue surgery reduces the chances of recurrence in both the benign and the malignant diseases. It is important to remove all functioning thyroid tissue especially in the differentiated thyroid cancer so the postoperative radioiodine ablation is more effective and serum thyroglobulin acts as an efficient marker.

Large defect of the lip and buccal mucosa created due to wide resection of the cancer is difficult to reconstruct and always a challenge. Such defects are complex as there is loss of outer skin, muscle, soft tissue, and inner mucosal lining. We report the use of submental artery island flap (SAIF) in the reconstruction of a large defect of oral commissure and buccal mucosa in a 70-year-old female with the diagnosis of verrucous carcinoma.

Although lipoma is frequently seen in the head and neck region, but occurrence of the lipoma in the parapharyngeal space is rare. It is extremely rare for a parapharyngeal lipoma to extent to the pterygoid region (ie, anterior skull base). Management of giant parapharyngeal space lipoma with skull base extension and proximity to the vital neurovascular structures poses a challenge to surgeon.

Objective: To analyze various demographic, clinical, and histopathologic factors in T1-2 N0 squamous cell carcinoma (SCC) of the oral tongue to define a high-risk group for regional recurrence that will benefit from elective neck dissection.

Materials And Methods: Retrospective outcome analysis of a patient cohort without palpable or ultrasound (USG) detectable nodal metastases undergoing per oral wide glossectomy for T1-2 N0 SCC of oral tongue. Patients were followed up using palpation and serial USG neck and fine-needle aspiration cytology.

Unilateral phrenic nerve palsy as initial presentation of the retrosternal goitre is extremely rare event. This is a case report of a 57-year-old woman with history of cough and breathlessness of 3 months duration, unaware of the thyroid mass. She had large cervico-mediastinal goiter and chest radiograph revealed raised left sided hemidiaphragm.

Riedel's thyroiditis is a rare inflammatory process which not only involves thyroid gland but also the surrounding vital structures. It may also be associated with various forms of systemic fibrotic disorders. The exact etiology is not known, but currently most favored view is that of a localized form of systemic fibrotic process.

Parathyroid adenoma usually presents with the signs and symptoms of hypercalcemia due to hyperparathyroidism. Brown's tumor is the late clinical consequence of untreated hyperparathyroidism and is rarely seen now-a-days due to improved screening biochemical tests. These tumors are characterized by high orthoclastic activity and bone resorption.

Clear cell myoepithelioma arising from the minor salivary glands of hard palate is a rare entity. Most of the cases of palatal myoepitheliomas reported so far are either plasmacytoid or spindle cell type. Our literature search revealed only one case report of clear cell myoepithelioma of the palate.

Juvenile ossifying fibroma (JOF) is an uncommon, benign, bone-forming neoplasm seen in the craniofacial bones. It is distinguished from other fibro-osseous lesions primarily by its age at onset, clinical presentation, and potential aggressive behavior. Occasionally JOF may grow aggressively and extend to involve the orbits and skull base, resulting in serious cosmetic and functional problems.

Article Synopsis
  • The study aimed to compare the outcomes of trans-oral CO2 laser microsurgery for early glottic squamous cell cancer in cases with and without anterior commissure involvement.
  • A total of 296 cases were analyzed, revealing higher local recurrence rates (29.51%) in patients with anterior commissure involvement compared to 18.3% in those without, with significant statistical differences.
  • Overall survival rates were similar between both groups, indicating that laser microsurgery is an effective treatment for early glottic cancer regardless of anterior commissure involvement, but requires skilled surgical techniques for optimal results.

Objectives/hypothesis: The purpose of the study was to study the outcomes of per oral wide excision of early T1-2 N0 squamous cell cancer of buccal mucosa.

Methods: This is a retrospective study of early and localized squamous cell cancer of the buccal mucosa treated with peroral wide excision at a tertiary-care hospital.

Results: A total of 156 patients were analyzed.

Papillary thyroid cancer favors lymphatic spread both within the thyroid gland and to the cervical lymph nodes. Hematogenous spread rarely occurs in the lung, bones and brain. We report a case where metastatic nodules from papillary thyroid cancer were seen in the right aryepiglottic fold (supraglottis) and lateral wall of hypopharynx, an extremely rare event.

Juvenile ossifying fibroma is a rare fibro-osseous neoplasm seen in the pediatric age group of 5 to 15 years. It has been recognized as a separate histopathologic entity among the fibro-osseous group of lesions. Occasionally, it may grow aggressively and extend to involve the orbits and skull base, resulting in serious cosmetic and functional problems.

Background: World literature suggests parapharyngeal space lesions account for only 0.5% head and neck tumours and the majority of the minor salivary gland tumours are malignant. The parapharyngeal space is very rare site for this tumour.
