Purpose: Non-optimal vaginal microbiota lacking lactobacilli and comprising a wide array of anaerobic bacteria, typified by community state type (CST) IV, have been associated with adverse gynecological and pregnancy outcomes. Here, we investigate the stability of the vaginal microbiota sampled every 6 months over 18 months and how samples distantly collected combined with exposures could provide insight on future microbiota compositional changes.

Methods: Vaginal microbiota dynamics were analyzed in 241 female students aged 18-24 years and negative for Chlamydia trachomatis and Neisseria gonorrhoeae.

Research Question: What is the impact of ethanol sclerotherapy of endometriomas prior to IVF/ICSI on pregnancy rates?

Design: We reviewed women with endometrioma(s) larger than 25 mm having IVF/ICSI cycles. All patients with a history of ovarian cystectomy were excluded. Two groups were compared: patients who had transvaginal ethanol sclerotherapy of their endometrioma(s) before ovarian stimulation and patients whose endometrioma(s) were left in situ (untreated) at the time of IVF/ICSI (control group).

Background: Recent recommendations from the French High Authority of Health on pelvic organ prolapse (POP) management underline the value of a pelvic examination.

Objectives: The aim of this paper was to analyze the literature and identify the best evidence available regarding pelvic examination for women presenting prolapse-associated symptoms in terms of diagnosis and predictability of treatment success.

Search Strategy: The databases were queried similarly using Medical Subject Headings (MeSH) and non-MeSH terms broadly related to pelvic examination and POP management.

Research Question: How do different warning indicators help to identify disabling dysmenorrhoea among women in young adulthood?

Design: A nationwide cross-sectional study of women aged 18-25 years from the CONSTANCES cohort was constructed. Disability was assessed with the Global Activity Limitation Indicator question 'For the past 6 months, have you been limited in routine activities?Yes, severely limited/Yes, limited/ No, not limited'. Dysmenorrhoea pain intensity and other chronic pelvic pain symptoms (dyspareunia and non-menstrual pain) were evaluated according to questions from a specific questionnaire.

Objectives: To describe the real-world use of Elastic Venous Compression Devices (EVCDs) during pregnancy and post-partum using data from a representative subset of the French National Health Insurance Claims Database (the Echantillon Généraliste des Bénéficiaires, EGB).

Study Design: Women aged 15-49 who were pregnant between 1st July 2017 and 15th June 2018 were identified in the EGB using pregnancy-specific acts (certain prenatal examinations or deliveries). Subgroups were defined by age, presence of Venous Thrombo-Embolism (VTE) risk factors, history of VTE, delivery type and time period.

Article Synopsis
  • - The study aimed to evaluate patient-reported outcome measures (PROMs) for assessing sexual function in women with endometriosis, focusing on their reliability and validity to enhance research and clinical practices
  • - A systematic review from January 2000 to September 2023 examined 74 studies on sexual function in endometriosis, identifying 25 different PROMs, with the Female Sexual Function Index (FSFI) being the most used
  • - Findings revealed that key measurement properties like "hypothesis testing" and "responsiveness" were most common among PROMs, with the FSFI and Sexual Activity Questionnaire being particularly valuable for evaluating medical and surgical treatments respectively

Objective: To analyze the literature and expose best evidence available regarding the benefit of pelvic examination for women with suspected endometriosis METHODS: the AGREE II and GRADE systems for grading scientific evidence.

Results: Endometriosis is characterized by the heterogeneity in its clinical presentation with many different symptoms reported by patients. In the literature, questioning for each symptom has a high sensitivity, reaching 76-98 %, but lacks specificity (20 - 58 %).

Background: Midurethral slings are the gold standard for treating stress urinary incontinence, but their complications may raise concerns. Complications may differ by the approach used to place them.

Objective: This study aimed to compare serious complications and reoperations for recurrence after midurethral sling procedures when using the retropubic vs the transobturator route for female stress urinary incontinence.

Objectives: Vaginal laxity concerns 24 to 38% of women but it's still poorly understood and studied. The objective of this study is to do an inventory of current scientific knowledge about its definition, its diagnostic criteria, and treatments.

Methods: We conducted a non systematic review of literature including original articles in French and English about the definition, diagnostic criteria and treatments of vaginal laxity using data bases such as Cochrane, Embase, Medline, PubMed et Science Direct.

Article Synopsis
  • The study aimed to evaluate different treatment options for women diagnosed with interstitial pregnancy (IP) by analyzing outcomes from various therapeutic methods between January 2008 and December 2019.
  • Three treatments were assessed: surgical treatment (ST), in situ methotrexate combined with systemic methotrexate (IS-MTX), and systemic methotrexate alone (IM-MTX). Success rates differed significantly, with ST showing a 100% success rate, while IS-MTX and IM-MTX had 70.6% and 31% success rates, respectively.
  • The findings suggest that ST and IS-MTX are effective for IP, while IM-MTX is less effective. Emergency surgery is recommended

Article Synopsis
  • - The study aims to create a standardized set of patient-centered outcomes for treating endometriosis, enhancing personalized care and monitoring treatment effectiveness.
  • - Using a modified Delphi method, 51 participants from 16 countries evaluated numerous Patient Reported Outcome Measures (PROMs) and Clinician Reported Outcome Measures (CROMs) to establish relevant metrics for routine care.
  • - The final outcomes include six PROMs focused on symptoms, pain, productivity, and quality of life, alongside ten CROMs covering clinical indicators; additional tools are also suggested for assessing psychological and sexual impacts of endometriosis.

  • The objective of the guidelines is to standardize the pelvic clinical exam for gynecology and obstetrics, ensuring practices are evidence-based.
  • A committee of 45 experts, including patient representatives, developed these guidelines independently, focusing on high-quality recommendations using the GRADE® system.
  • They formulated 40 questions into a PICO format, yielding 27 recommendations, with differing levels of agreement, while highlighting the need for more research in areas lacking evidence.

  • Preoperative mapping of deep pelvic endometriosis (DPE) is essential for successful surgery, with quality MRI information being critical for planning.
  • A study involving 605 women analyzed MRI scans to evaluate the Deep Pelvic Endometriosis Index (dPEI) score, revealing that a majority had mild to moderate endometriosis, with severe cases linked to longer surgery times and hospital stays.
  • Patients with severe DPE faced significantly higher chances of severe postoperative complications and issues like voiding dysfunction compared to those with mild or moderate cases.

Objective: Preoperative assessment of rectal damage in digestive endometriosis requires rectal endoscopic ultra-sonography an invasive exam that is not well received by the patients. A standardized approach using an Ultrasound-Based Endometriosis Staging System (UBESS)could be an interesting tool in this indication. This article aims to evaluate the performance of UBESS in the prediction of rectal involvement and the type of surgical procedure.
