Background/objectives: Positive pancreatic juice cytology (PJC) is an important finding when considering surgical resection in patients with intraductal papillary mucinous neoplasm (IPMN); however, guidelines do not recommend endoscopic retrograde cholangiopancreatography (ERCP) for PJC. This study aimed to clarify the findings worthy of adding PJC for diagnosing high-grade dysplasia (HGD) and invasive carcinoma (IC) in patients with IPMN.

Methods: Patients with IPMN who underwent preoperative PJC and surgical resection at Hiroshima University Hospital were enrolled, and the diagnostic yield of malignant IPMN based on PJC and clinical and imaging findings and the incidence of post-ERCP pancreatitis (PEP) were retrospectively analyzed.

Article Synopsis
  • The study aimed to assess endoscopic resection methods for cT1b colorectal carcinomas (CRCs) measuring ≤20 mm, focusing on achieving sufficient vertical margins (VMs).
  • Researchers evaluated 128 patients, categorizing tumor lifting conditions into types A (soft dome), B (hard trapezoid), and non-lifting, where they found that all non-lifting tumors required hybrid endoscopic submucosal dissection (ESD) to ensure VMs of at least 500 µm.
  • The results indicated that hybrid ESD is more effective for type B tumors, producing significantly better VM outcomes compared to endoscopic mucosal resection.

Background/objectives: Colorectal neoplasia developing from ulcerative colitis mucosa (CRNUC) can be divided into ulcerative colitis-associated neoplasia (UCAN) and non-UCAN; however, it is often difficult to distinguish UCAN from non-UCAN during a biopsy diagnosis. We investigated whether a genomic analysis could improve the diagnostic accuracy of UCAN using biopsy specimens.

Methods: In step 1, 14 CRNUCs were used to examine whether the genomic landscape of biopsy and resection specimens matched.

Background/aim: Combined hepatocellular cholangiocarcinoma (cHCC-CCA) is a rare subtype of primary liver carcinoma, characterized by the unequivocal presence of both hepatocellular carcinoma (HCC) and intrahepatic cholangiocarcinoma (iCCA). However, its clinicopathological characteristics have not yet been thoroughly elucidated. In particular, cholangiolocellular carcinoma (CLC) was classified as a subtype of cHCC-CCA according to the 2010 World Health Organization (WHO) classification.

  • A study investigated how the timing of endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) affects peritoneal lavage cytology (CY) outcomes in patients with resectable pancreatic body-tail cancer.
  • Among 129 patients, 12% tested positive for CY, with lymph node metastasis being the only significant risk factor identified.
  • The timing of EUS-FNA, whether within 14 days or 15 days and beyond, did not show a significant difference in CY positivity rates, suggesting EUS-FNA timing might not influence outcomes.

  • Familial adenomatous polyposis (FAP) is a hereditary condition leading to hundreds or thousands of colorectal polyps, which can progress to cancer if untreated; it’s caused by mutations in the APC gene.
  • The report discusses two cases where women with FAP experienced an increase in fundic gland polyps after using proton pump inhibitors, which were then reduced upon switching medications.
  • The study concludes that these types of medications can raise gastrin levels, leading to a rapid increase in polyps, and highlights the need for careful prescription of these drugs in FAP patients.

Background: Tumors arising from catecholamine-producing chromophil cells in paraganglia are termed paragangliomas (PGLs), which biologically resemble pheochromocytomas (PCCs) that arise from the adrenal glands. Spontaneous rupture of a PGL is rare and can be fatal. Although elective surgery for ruptured PCCs after transcatheter arterial embolization (TAE) has been shown to provide good outcomes, the efficacy of TAE pretreatment for ruptured PGL remains unknown.
