Changes in the macromolecular proteolycan (PG) and collagen of the cartilage matrix may culminate in irreparable tissue destruction. Molecular modifications appear to result from: (A) exogenous proteinases, (B) endogenous chondrocyte proteinases whose synthesis and release is modulated by exogenous non-enzymatic cytokines (CKs) and (C) quantitative and/or qualitative alterations in chondrocyte PG and collagen synthesis which are potentially induced by exogenous CKs. Studies have recently been initiated to determine the effect of piroxicam on the synthesis and activity of such metabolic regulatory CKs in patients with rheumatoid arthritis and osteoarthritis, and in age-, sex-, and race-matched controls.

This paper describes four patients with undetected progressive supranuclear palsy who were referred to psychiatric facilities for treatment of late onset symptoms and signs attributed to primary psychopathology. However, subsequent neurological and neuropsychological evaluation established the diagnosis of progressive supranuclear palsy. In two patients, psychiatric symptoms preceded the onset of neurological deficits.

Nonenzymatic, trypsin sensitive cytokines derived from lectin stimulated normal human mononuclear cells have been shown to induce release of proteoglycan and collagen degrading proteinase activity from chondrocytes in cartilage organ and isolated suspension culture systems. Active chondrocyte protein and RNA synthesis were required to induce activity. Cytokines responsible were of both monocyte and T cell origin.

Although acute renal failure, caused either by renal ischemia or nephrotoxic agents, is usually characterized by oliguria, a severe fall in glomerular filtration rate, and a fall in renal blood flow, some patients and experimental models display a non-oliguric pattern of renal injury. The present study was designed to evaluate the mechanism of preservation of high urinary flow rate under this condition. Following the administration of the aminoglycoside gentamicin to rats for five days, a decrease in concentrating ability was demonstrated, caused by impaired vasopressin-mediated water transport.

The cell-free synthesis of hyaluronic acid has been demonstrated in extracts of cultured human fibroblasts. Preparations from fibroblasts of normal individuals as well as those from patients with Marfan syndrome incorporate glucuronic acid and N-acetylglucosamine from their UDP derivatives into hyaluronic acid. Extracts from Marfan fibroblasts demonstrate 3 to 10 times more total and specific hyaluronic acid synthetase activity than do preparations from normal fibroblasts.

A case of esophageal Crohn's disease occurring in a 40-year-old woman with proctocolitis and oral manifestation is reported. A possible connection between Crohn's disease and Behçet's syndrome is discussed. A survey of the literature indicates that comparable lesions occur very rarely.

In treatment of malignant lymphomas exploratory laparotomy with splenectomy, liver and lymphnodes biopsy is necessary in order to individualize appropriate management. The surgeon must make maximal effort to obtain lymphnodes for histological examination. The surgical anatomy of the retroperitoneum in space of abdominal aorta and the biopsy procedure of paraaortic lymphnodes were described.

A prospective study was carried out in 25 patients with systemic lupus erythematosis (SLE) on the effect of normalizing serum complement (CH50) and anti-DNA antibodies on the course of lupus nephritis. In 16 of the 25 patients, CH50 was maintained within the normal range for two years. Urinary protein excretion increased or remained low in all 16.

An experimental study is reported on different injections of barium sulfate into peritoneal cavity of 10 dogs and intravasal. All experimental animals died. The experiments show that barium sulfate causes not only local reaction but the most important is it's general deleterious effect.
