Entrustable Professional Activities (EPAs) are an important tool to support individualisation of medical training in a competency-based setting and are increasingly implemented in the clinical speciality training for endocrinologist. This study aims to assess interrater agreement and factors that potentially impact EPA scores. Five known factors that affect entrustment decisions in health profesions training (capability, integrity, reliability, humility, agency) were used in this study.

Objective: Thyroglobulin (Tg) is an established tumor marker for differentiated thyroid carcinoma (DTC) patients. However, Tg immunoassays can be subject to Tg autoantibody (TgAb) interference resulting in incorrect Tg values. Therefore, Tg measurement with liquid chromatography-tandem mass spectrometry (LC-MS/MS) could be promising in patients with TgAbs.

Objective: Hemangioblastomas in the central nervous system are the most common manifestation of von Hippel-Lindau (VHL) disease. Because the growth rate of hemangioblastomas is unpredictable, regular follow-up is mandatory, focusing on clinical symptoms and imaging of the central nervous system. However, clinical symptoms may be subtle and nonspecific, and data about the relationship between the radiologic findings and clinical symptoms are sparse.

Background: Measurement of the 24-h urinary iodine concentration or urinary iodine excretion (UIE) is the gold standard to determine iodine status; however, this method is inconvenient. The use of salivary iodine could be a possible alternative since salivary glands express the sodium-iodine symporter.

Objectives: We aimed to establish the correlation between the salivary iodine secretion and UIE, to evaluate the clinical applicability of the iodine saliva measurement.

Article Synopsis
  • The study investigates long-term male fertility in men treated with radioactive iodine (RAI) for differentiated thyroid carcinoma (DTC), particularly analyzing semen quality before and after RAI.
  • Conducted as a multicenter research project, it involved 51 males evaluated at least two years post-RAI treatment, focusing on semen analysis and hormonal evaluation.
  • Results showed that the semen quality of participants was normal and comparable to the general population, indicating that routine cryopreservation may not be necessary for fertility post-RAI, though it could be beneficial in specific situations.

Article Synopsis
  • - The study examines bone mineral density (BMD) in pediatric survivors of differentiated thyroid carcinoma (DTC) who have undergone thyrotropin-suppressive therapy, focusing on the potential long-term effects of subclinical hyperthyroidism and permanent hypoparathyroidism.
  • - Out of 65 participants, 29% exhibited subclinical hyperthyroidism; while most maintained normal BMD, 13% displayed low BMD after 23.5 years of follow-up, indicating some risk among long-term survivors.
  • - The study found that BMD remained stable over time for both those with and without permanent hypoparathyroidism, as bone turnover markers also showed consistent results throughout the follow-up period.

Background: Despite adequate presurgical management, blood pressure fluctuations are common during resection of pheochromocytoma or sympathetic paraganglioma (PPGL). To a large extent, the variability in blood pressure control during PPGL resection remains unexplained. Adrenomedullin and B-type natriuretic peptide, measured as MR-proADM and NT-proBNP, respectively, are circulating biomarkers of cardiovascular dysfunction.

Background: A diagnostic I-131 (Dx) scan is used to detect a thyroid remnant or metastases before treatment of differentiated thyroid cancer (DTC) with I-131. The aim of this study is to specify in which patients with DTC a Dx scan could have an additional value, by studying the effect of the Dx scan on clinical management.

Methods: Patients with DTC, treated with I-131 after thyroidectomy were included in this retrospective cohort study.

Article Synopsis
  • The study evaluates the use of salivary metanephrines for diagnosing pheochromocytomas and paragangliomas (PPGL), highlighting its noninvasive collection method compared to plasma or urine samples.
  • Results show that salivary metanephrines in patients with PPGL are significantly higher than in healthy participants, with a diagnostic sensitivity of 89% and specificity of 87%.
  • Despite promising results indicating a correlation between salivary and plasma metanephrines, salivary testing is not yet a replacement for traditional plasma measurements in diagnosing PPGL.

Background To diagnose pheochromocytoma or sympathetic paraganglioma, guidelines recommend blood sampling after at least 30 min of supine rest and using an indwelling intravenous cannula is preferred. Although blood sampling by venipuncture is more convenient and cost-effective, it is unknown whether venipuncture affects plasma concentrations of free metanephrines (MNs). We therefore investigated whether there is a difference in plasma concentrations of free MNs collected by venipuncture or by an intravenous cannula.

Context: Current guidelines do not consistently recommend imaging beyond the head and neck region in succinate dehydrogenase subunit D (SDHD) mutation carriers as long as catecholamine metabolite levels are within the reference range.

Participants: We report a series of 10 patients carrying pathogenic variants in the SDHD gene from five tertiary referral centers for paraganglioma (PGL) in the Netherlands, who presented with a sympathetic PGL (sPGL), pheochromocytoma (PHEO), or metastases outside the head and neck region in the absence of excessive catecholamine production. Two of six patients with a biochemically silent sPGL/PHEO developed metastatic disease.

Context: F-FDOPA PET/CT accurately localizes pheochromocytoma in patients with an established biochemical diagnosis. However, cut-off F-FDOPA levels of standardized uptake values (SUV) for both normal adrenal glands and pheochromocytoma are lacking.

Objective: Objectives of this study were to determine (1) reference maximum standardized uptake values (SUVmax) for normal adrenal F-DOPA tracer uptake and (2) the optimal diagnostic approach for pheochromocytoma localization by using F-DOPA SUVmax across a series of cut-off points: the affected adrenal gland (inter-individual analysis), the difference in SUVmax between the affected adrenal gland and the contralateral normal adrenal gland (intra-individual analysis), or a combination of these two.

Succinate dehydrogenase (SDH) mutations lead to the accumulation of succinate, which acts as an oncometabolite. Germline SDHx mutations predispose to paraganglioma (PGL) and pheochromocytoma (PCC), as well as to renal cell carcinoma and gastro-intestinal stromal tumors. The SDHx genes were the first tumor suppressor genes discovered which encode for a mitochondrial enzyme, thereby supporting Otto Warburg's hypothesis in 1926 that a direct link existed between mitochondrial dysfunction and cancer.

Germline mutations in succinate dehydrogenase subunit B and D ( and ) are predisposed to hereditary paraganglioma (PGL) and pheochromocytoma (PHEO). The phenotype of pathogenic variants varies according to the causative gene. In this retrospective study, we estimate the mortality of a nationwide cohort of variant carriers and that of a large cohort of variant carriers and compare it to the mortality of a matched cohort of the general Dutch population.

Article Synopsis
  • Some doctors use a special scan called F-FDOPA PET to find certain tumors called neuroendocrine tumors (NETs).
  • This study looked at many patients who had this scan to see if it sometimes showed false positives (meaning it looked like there was a tumor when there wasn't).
  • They found that some patients had tumors that weren't NETs, like other types of cancer, even though the scan showed there might be one.
