Background And Objectives: Many neonatal intensive care units (NICUs) do not give rotavirus vaccines to inpatients due to a theoretical risk of horizontal transmission of vaccine strains. We aimed to determine incidence and clinical significance of vaccine-strain transmission to unvaccinated infants in a NICU that routinely administers pentavalent rotavirus vaccine (RV5).

Methods: This prospective cohort study included all patients admitted to a 100-bed NICU for 1 year.

Article Synopsis
  • * Analyzed data indicated that areas with higher social vulnerability experienced significantly more emergency dental visits and inpatient stays compared to less vulnerable areas, highlighting a correlation between social challenges and dental health issues.
  • * Increased availability of dental providers was linked to fewer emergency visits, suggesting that improving access to dental care could alleviate the pressure on emergency services in vulnerable communities.

The oxygen reduction reaction (ORR) is important for alternative energy and industrial oxidation processes. Herein, an iminium-based organoelectrocatalyst () for the ORR with trifluoroacetic acid as a proton source in acetonitrile solution under both electrochemical and spectrochemical conditions using decamethylferrocene as a chemical reductant is reported. Under spectrochemical conditions, HO is the primary reaction product, while under electrochemical conditions HO is produced.

Photosensitivity to structurally diverse drugs is a common but under-reported adverse cutaneous reaction and can be classified as phototoxic or photoallergic. Phototoxic reactions occur when the skin is exposed to sunlight after administering topical or systemic medications that exhibit photosensitizing activity. These reactions depend on the dose of medication, degree of exposure to ultraviolet light, type of ultraviolet light, and sufficient skin distribution volume.

Importance: Variability in intervention participation within care management programs can complicate standard analysis strategies.

Objective: To evaluate whether care management was associated with reduced hospital readmissions among individuals with higher participation probabilities.

Design, Setting, And Participants: A total of 800 hospitalized patients aged 18 years and older were randomized as part of the Health Care Hotspotting randomized clinical trial, which was conducted in Camden, New Jersey, from June 2014 to September 2017.

This article describes the use of USS or CT guided hook-wire localization to aid the excisional biopsy of non-palpable lymph nodes in the cervical, axillary and inguinal regions. This technique allows a more focussed surgical approach intra-operatively to reduce surgical time and morbidity for the patients.

Unlabelled: Financial stress and lack of financial well-being are significant contributors to physician burnout. Many trainees believe little can contribute to developing financial freedom during their training years. However, residency is a pivotal moment in a young attending's life; strategic financial steps taken during this time can lead to a path of financial freedom and well-being for years to come.

Introduction Insight into the characteristics of populations from which research samples are drawn is essential to understanding the generalizability of research findings. This study characterizes the membership of Kaiser Permanente and compares members to the population of the communities in which they live. Methods This study is a descriptive comparison of population distributions for Kaiser Permanente members vs the general population within counties in which Kaiser Permanente operates.

The presence of another individual may increase or decrease the likelihood a person will use drugs, depending on factors such as whether the source (i.e., the other individual) is also using drugs.

Article Synopsis
  • The study focuses on Black/African American women smokers in rural areas with limited healthcare access, particularly examining the prevalence of smoke-free home rules among this population.
  • It analyzes various sociodemographic factors and smoking behaviors, revealing that only 26% of women enforced a complete ban on smoked tobacco in their homes.
  • Financial security plays a significant role, with women facing economic challenges more likely to lack a complete ban, suggesting that perceived financial stability may encourage stricter smoke-free policies at home.

Insulin-secreting β-cells are functionally heterogeneous. Whether there exist cells driving the first-phase calcium response in individual islets, has not been examined. We examine "first responder" cells, defined by the earliest [Ca2+] response during first-phase [Ca2+] elevation, distinct from previously identified "hub" and "leader" cells.

Objective: To introduce a novel analytical approach for randomized controlled trials that are underpowered because of low participant enrollment or engagement.

Data Sources: Reanalysis of data for 805 patients randomized as part of a pilot complex care intervention in 2015-2016 in a large delivery system. In the pilot randomized trial, only 64.

Summary: Adrenal gland haemorrhage is an uncommon, yet likely under-diagnosed complication of high-impact trauma, such as motor vehicle accidents (MVA). It usually occurs with multi-trauma and is associated with additional injuries to the ribs, liver, kidney, spleen and vertebrae. Trauma cases with resultant adrenal gland injury have higher mortality rates.

Primary cutaneous cribriform apocrine carcinoma (PCCAC) is an exceedingly rare sweat gland carcinoma. Clinically, it most often presents as a singular, asymptomatic nodule or cyst on the extremities of middle-aged patients and follows an indolent course. Both visceral and cutaneous cribriform tumors exist in nature.

Described are the first examples of Lewis acid-promoted Diels-Alder reactions of vinylpyridines and other vinylazaarenes with unactivated dienes. Cyclohexyl-appended azaarenes constitute a class of substructures of rising prominence in drug discovery. Despite this, thermal variants of the vinylazaarene Diels-Alder reaction are rare and have not been adopted for synthesis, and Lewis acid-promoted variants are virtually unexplored.

Community health workers (CHWs) are critical to health equity efforts, but sustaining CHW programs is challenging. Understanding stakeholders' knowledge and attitudes about CHWs can inform strategies to advance this important workforce. The authors implemented an online survey of potential CHW employers to learn their perceptions of CHWs' roles, outcomes, and abilities to affect important health outcomes, and of key issues that affect CHW employment.

3,4-Methylenedioxymethamphetamine (MDMA) is classified as an entactogen, producing feelings of emotional openness and relatedness. One unique feature of MDMA is that people tend to selectively take this drug in social and/or intimate situations. Although MDMA is recognized as having abuse liability, preclinical studies report that it has weak reinforcing effects in animals.

Introduction: The design of sensors that can detect biological ions remains challenging. While many fluorescent indicators exist that can provide a fast, easy readout, they are often nonspecific, particularly to ions with similar charge states. To address this issue, we developed a vesicle-based sensor that harnesses membrane channels to gate access of potassium (K) ions to an encapsulated fluorescent indicator.

A mild, broadly functional group tolerant methodology has been developed to access a variety of mono- and bis-carbodiimides in good yield and high purity on multigram scale. Direct addition into these versatile motifs facilitated the rapid synthesis of a library of novel amidinines, guanidines, and phosphaguandines.

Objective: Research is needed on disaster-affected workplaces, particularly on employee job satisfaction and performance, to inform workplace responses promoting employee postdisaster adjustment and wellbeing.

Methods: Quantitative and qualitative data were collected from a volunteer sample of 255 employees of eight workplaces affected by the 9/11 attacks on New York City's World Trade Center nearly 3 years post disaster.

Results: The effects of 9/11 on both job satisfaction and job performance were more negative than positive, especially for the Ground Zero employees.
