Aortic arch thrombosis (AAT) is a potentially life threatening condition in neonates. We report 3 neonates with AAT and complete occlusion of a unilateral internal carotid artery with multiple cerebral infarcts. Multidisciplinary input led to deferral of thrombolytic therapy and surgical thrombectomy owing to the potential risk of hemorrhage within the infarcts.

Background: Development of post-thrombotic syndrome (PTS) is increasingly being recognized as a complication of deep venous thrombosis (DVT) in children.
Objective: To determine the prevalence, clinical characteristics, and predictors of moderate to severe PTS in children.
Methods: A retrospective chart review was performed on those children who were followed in the coagulation clinic for objectively confirmed DVTs from December 2004 to December 2006.
Treatment of thrombosis in children with end-stage renal disease (ESRD) is extremely challenging owing to the underlying risk of bleeding. Fondaparinux (Arixtra, Sanofi-Synthélabo), a synthetic pentasaccharide, is contraindicated in patients with compromised renal function as it is excreted via kidneys. We describe a unique case with ESRD and pulmonary embolism who was treated with fondaparinux owing to the toxicity and poor compliance with low-molecular-weight heparin.
