Common bile duct duplications represent exceptionally rare congenital anomalies of the biliary tract. In this case report we document an unusual variant of common bile duct duplication in a 79-year-old man who underwent a pancreaticoduodenectomy for ampullary cancer. The duplication consisted of two unseparated, completely-layered, common bile ducts which originated above the cystic duct junction and terminated prior to the point of insertion into the pancreas, where the two lumens converged into a single duct.

Article Synopsis
  • This study compares three cytological classification systems (C1-C5, Papanicolaou, and WHO) for evaluating pancreatic neuroendocrine lesions in 73 resections over 12 years at a UK center.
  • Results showed that the WHO classification had the highest accuracy in categorizing these lesions, while the Papanicolaou system excelled in differentiating neuroendocrine tumors from more aggressive forms like pancreatic adenocarcinoma.
  • Overall, the study suggests that while the WHO system reduces ambiguity in malignancy classification, the Papanicolaou system provides practical management benefits without sacrificing accuracy.

Article Synopsis
  • Anti-PD-1 therapy has been newly approved by NICE for adults with recurrent and metastatic head and neck squamous cell carcinomas (HNSCC), highlighting the emerging role of immunotherapy in treating these cancers.
  • This study aimed to review PD-L1 testing results from HNSCC cases alongside other tumor types and assess how these findings could be integrated into clinical practice for better biomarker utilization.
  • Over five years, PD-L1 testing was performed on 199 cancer cases, revealing that while only a few showed positivity, immunotherapy still led to notable improvements in overall survival for patients with PD-L1 positive tumors.

Article Synopsis
  • - Hepatocellular carcinoma (HCC) is a major cause of cancer deaths globally, and the study focuses on the decreasing role of alpha-fetoprotein (AFP) as a biomarker compared to imaging studies and new biomarkers.
  • - The research involved 131 newly diagnosed HCC patients, analyzing the relationship between elevated AFP levels and various tumor and liver features, using a cut-off level of 200 IU/mL.
  • - Results indicated that a tumor size greater than 5 cm is a significant predictor of elevated AFP levels, along with other factors like tumor type and portal vein thrombosis, suggesting that AFP may still be a useful tool in certain contexts, particularly in developing countries.
