Comparison of recording positions of physical activity in patients with severe COPD undergoing LTOT.

Background: Accelerometry is increasingly used to assess physical activity (PA) in patients with chronic obstructive pulmonary disease (COPD). It is not known how the relationship of PA to clinical results depends on the position of the PA sensor.

Methods: We assessed the effect of monitor position by measuring lower extremity (ankle), upper extremity (wrist) and total body movement (hip) in 52 patients with severe COPD (mean [± SD] age, 62 ± 10 years; FEV(1), 38 ± 12% predicted) undergoing long-term oxygen therapy with and without walkers during a pulmonary rehabilitation (PR) program.

Minute type I gastric carcinoid with regional lymph node metastasis.

Type I gastric carcinoids are indolent lesions that only rarely metastasize. Until now, lymph node metastasis had only being observed in large (≥10 mm) and usually invasive lesions (deep submucosa or beyond). This study reports a diminutive (8 mm) gastric carcinoid, limited to the superficial layer of the submucosa (sm1), with no angiovascular invasion, but with a high Ki-67 index (7%) and an involved perigastric lymph node.

Modified supralevator pelvic exenteration for the treatment of locally advanced rectal cancer with vaginal and uterine invasion.

Post-chemoradiation T4 rectal cancer remains a therapeutic challenge and an aggressive surgical approach is the only chance for a cure. Rectal lesions infiltrating the upper vaginal wall and uterine cervix are usually treated by low anterior resection with en bloc removal of the vagina and uterus. However, failure can occur when one is trying to access the anterior recto-vaginal plane below the tumor, especially in obese patients with a narrow pelvis.

Exploring the community structure of a diabetes forum.

A health forum is a kind of social network where users share information for specific topics they create. The purpose of this study was the identification of the key actors and the user communities in such a network. We used the publicly available data from a diabetes forum to create the corresponding network and explore several algorithms for the detection of user communities.

Scale-space methods for live processing of sensor data.

A temporal scale-space is a vector space spanned by time and a scale parameter, and by constructing the scale-space correctly a causal structure can be imposed on the scale-space. This enables early warning of significant changes in sensor data at an early time, and on any scale. We describe a feasibility study on how to use these ideas for live surveillance of monitoring processes such that important features can be visualized and users warned about changes an early stage.

Prevalence of conduction abnormalities in a systolic heart failure population by race, ethnicity, and gender.

Background: There is paucity of data regarding conduction abnormalities in the Hispanic population with systolic heart failure (HF). We aimed to evaluate the prevalence of electrocardiogram (ECG) abnormalities in a systolic HF population, with attention to the Hispanic population.

Methods: A cross sectional study of 926 patients enrolled in a systolic HF disease management program.

Isolated splenic metastasis from lung squamous cell carcinoma.

Isolated splenic metastasis from lung cancer is a very rare occurrence with only a few reports available. Here, we report the case of a 82-year-old male who underwent a bilobectomy for a lung squamous cell carcinoma and 16 months later developed an isolated splenic metastasis. Additionally, previous reports are reviewed and discussed.

Determination of 1,25-dihydroxyvitamin D concentrations in human colon tissues and matched serum samples.

Background: A novel method to measure 1,25-dihydroxyvitamin D (1,25(OH)(2)D) in human tissue was developed and validated. The objectives of this study were to determine whether 1,25(OH)(2)D is present in human colon tissue and to characterize the relationship between human colon tissue and serum 1,25(OH)(2)D concentrations.

Materials And Methods: Normal colon tissue specimens and matched serum samples were obtained from 30 patients who had undergone colectomy.

Meta-evaluation of baseline studies of the Brazilian Family Health Strategy Expansion Project: a participatory and formative approach.

A participatory, formative meta-evaluation of baseline studies in Brazil is presented. International standards recommended by associations of evaluators were used, along with "specificity" criteria built up using the terms of reference for proposals for the selection of studies. The methodological approach combined a "peer review" of baseline study reports, with a participatory (self) assessment for "primary" evaluators, the average of which provided the final score.

Introduction: In the past decade, there has been an increasing amount of published information regarding erectile dysfunction (ED) and heart failure (HF) in economically advanced, westernized populations. However, there is a paucity of data regarding ED and HF in developing countries. The country of Georgia is categorized as a lower-middle-income country (LMIC) with an emerging and developing economy.

Evidence-based medication adherence in Hispanic patients with systolic heart failure in a disease management program.

The Hispanic population is the fastest growing minority in the United States, yet there is a paucity of data regarding patient follow-up in heart failure disease management programs (HFDMPs) and evidence-based medication adherence. The purpose of this study is to measure the compliance of evidence-based medication use, specifically measuring angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) and beta-blockers (BBs) in the Hispanic population, and compare these data to the white and black population. The authors conducted a cross-sectional study of 561 patients enrolled in an HFDMP at Jackson Medical Hospital in Miami, Florida.

More than 5 million people live with heart failure (HF) in the United States, and this number is expected to rise due to several factors including increased life expectancy brought about by medical therapy and the aging of the population. HF and peripheral arterial disease (PAD) share many risk factors. A review of the literature reveals several studies supporting a higher prevalence of HF in patients with PAD than in those without PAD.

Aims: The epidemiology of the five stages of chronic kidney disease (CKD) in systolic heart failure (HF) patients has predominantly been described in hospitalized White patients, with little known about the prevalence in outpatient Blacks and Hispanics. Usually identified in middle age, the MetS has pediatric roots and there are variable incidence rates between ethnic groups. Due to the difficulty of diagnosis, it remains largely undetected in adolescents.

Plant species richness regulates soil respiration through changes in productivity.

Soil respiration is an important pathway of the C cycle. However, it is still poorly understood how changes in plant community diversity can affect this ecosystem process. Here we used a long-term experiment consisting of a gradient of grassland plant species richness to test for effects of diversity on soil respiration.

Effects of Korean red ginseng (Panax ginseng C.A. Mayer) and its isolated ginsenosides and polysaccharides on arterial stiffness in healthy individuals.

Background: Preclinical studies indicate a role of Korean red ginseng (KRG) in the modulation of vascular function; however, clinical evidence is scarce. Therefore, the objective of this study was to investigate the effect of KRG root on peripheral blood pressure (BP) and augmentation index (AI), an emerging method to assess cardiovascular risk beyond conventional BP measurements. Furthermore, in an attempt to elucidate which of the major components of KRG is responsible for these effects, the ginsenoside and polysaccharide fractions isolated from the same KRG root were also investigated.
