Article Synopsis
  • Peri-medullary arteriovenous fistula (PMAVF) is a rare spinal vascular condition that can cause serious neurological issues or bleeding in the spinal canal.
  • A case study of a 2-year-old boy with high-flow PMAVF showed significant improvement in his motor strength after a successful transarterial endovascular treatment targeting a single feeder artery.
  • PMAVFs typically feature multiple feeder arteries but this case was unique due to its single feeder artery and large venous pouch, highlighting the effectiveness of endovascular intervention as a primary treatment option for this type of malformation.

Background: Brain arteriovenous malformations (bAVMs) remains one of the most prevalent causes of intracranial hemorrhage and stroke-like syndromes in the young adult population. Although it has been agreed upon that definitive treatment using either single or multi-modal approach is warranted for successful bAVM management, much debate still revolves regarding the optimal timing of definitive treatment.

Case Summary: In this report, we present a case of delayed, definitive endovascular treatment for ruptured bAVM in a 21-year-old female, 3 mo post-ictus.

Background: Dopamine replacement therapy remains the gold standard for symptomatic management of Parkinson's disease worldwide. However, most patients will develop debilitating motor levodopa-induced complications (MLIC) in the form of levodopa-induced dyskinesia (LID) and/or motor fluctuations (MF). This study aimed to conduct a systematic review and meta-analysis on the pharmacogenetic association between LID and MF with common genetic variants of the dopamine metabolic and signaling pathways.
