Article Synopsis
  • - The study evaluated trends and outcomes of gestational carrier (GC) pregnancies in the U.S. using data from over 14 million deliveries between 2017 and 2020, finding that the prevalence of GC pregnancies rose by 55% during this period.
  • - Among GCs, there was a higher likelihood of complications such as multiple pregnancies, placental abruption, and low-lying placenta, along with increased risks for late-preterm deliveries and postpartum hemorrhage in singleton births compared to non-GCs.
  • - While GC pregnancies are relatively rare, the findings indicate they are becoming more common and generally have favorable pre-pregnancy characteristics, but their obstetric outcomes can be mixed, especially regarding certain complications. *

Coccidioides is a fungus endemic to the Southwestern United States, known for causing Valley Fever. While typically asymptomatic or presenting as a respiratory tract infection, rare cases of disseminated infection to the peritoneum have been reported. The patient in this case study presented with a small bowel obstruction, requiring an exploratory laparoscopy showing evidence of an inflammatory mass with a jejuna-colic fistula with adherence to the spleen.

  • Adenoma detection rate (ADR) is crucial for reducing colorectal cancer risk, with rates over 30% linked to lower cancer incidence.
  • A study compared screening colonoscopies before and after implementing the AI detection system (GI Genius) among five experienced endoscopists, showing a significant increase in ADR in a community setting.
  • The improvement in ADR was primarily due to better detection of small polyps, but the clinical importance and economic benefits of using AI in this context need further exploration.

  • Axillary lymph node dissection (ALND) is a surgical procedure used to treat malignant diseases, but it has known complications.
  • A lesser-known complication is a painless dermatomal rash called transient telangiectatic purpura.
  • This case report details a specific instance of transient telangiectatic purpura, exploring its development and suggesting possible underlying mechanisms.

Objective: To identify where reproductive endocrinology and infertility (REI) fellows trained for residency.

Design: Observational, cross-sectional study.

Setting: Not applicable.

Article Synopsis
  • The study aimed to explore how reproductive endocrinology and infertility specialists manage early pregnancy loss (EPL), focusing on the use of mifepristone.
  • A survey of 101 specialists revealed that most prefer medical management for EPL, but only 26.7% prescribe mifepristone due to access issues.
  • The research found that those prescribing mifepristone are typically in hospital or university settings, and experience level influences its usage, but factors like age, gender, and prior abortion training do not significantly affect it.

  • * The 1000 Genomes Project and Oxford Nanopore Technologies are working together to produce LRS data from at least 800 samples to enhance the identification of genetic variations and better understand human genetic diversity.
  • * Initial analysis of 100 samples shows high accuracy in detecting genetic variants, including structural variants that disrupt gene function, and provides valuable data for the clinical genetics community to advance research on pathogenic variations.

Importance: Chronic back pain (CBP) is a leading cause of disability. Placebo treatments often provide as much pain relief as bona fide treatments, such as steroid injections. Open-label (honestly prescribed) placebos (OLPs) may relieve CBP without deception, but OLP mechanisms remain poorly understood.

  • * Cervical cancer had the highest representation among AYA patients at 25.3%, with notable increases in annual rates for several female cancers, particularly colorectal cancer which saw the largest increase at 4.1%.
  • * The findings indicate a rising trend of AYA patients diagnosed with certain cancers related to obesity, especially in the three most common gynecologic cancers: ovary, uterus, and cervix.

Article Synopsis
  • Advancements in assisted reproductive technology have led to a rise in gestational carrier pregnancies, highlighting the need for more research on their perinatal outcomes and associated risks.
  • The objective of the study was to analyze maternal characteristics and obstetric outcomes in pregnancies involving gestational carriers.
  • A systematic review included six studies analyzing over 28,000 GC pregnancies, revealing higher rates of frozen embryo transfers and comparing various obstetric outcomes with non-GC pregnancies.

Oxytocin (OXT) is a peptide hormone and a neuropeptide that regulates various peripheral physiological processes and modulates behavioral responses in the central nervous system. While the humoral release occurs from the axons arriving at the median eminence, the neuropeptide is also released from oxytocinergic cell axons in various brain structures that contain its receptor, and from their dendrites in hypothalamic nuclei and potentially into the cerebrospinal fluid (CSF). Understanding oxytocin's complex functions requires the knowledge on patterns of oxytocinergic projections in relationship to its receptor (OXTR).

Bow Hunter's syndrome (BHS) is an uncommon condition characterized by impingement of one of the two vertebral arteries induced by cervical rotation, causing symptomatic vertebrobasilar insufficiency of the posterior cerebral circulation. We report a case of BHS in an 84-year-old male. Two months following a motor vehicle accident, the patient presented to an urgent care facility with subsequent transfer to the emergency department with complaints of lightheadedness upon right-lateral head movement.

  • Craniofacial injuries are common in children, and human birth tissue products like amniotic membranes offer potential for healing due to their growth factors and signaling molecules.
  • A 10-year-old girl with extensive soft tissue loss from a dog bite received a cryopreserved amniotic membrane graft as part of her surgical treatment.
  • Results showed successful tissue healing and epithelialization over time, suggesting that human birth tissue can effectively aid in recovery from acute facial injuries.

Background: Overprescription of opioids in the United States increases risks of opioid dependence, overdose, and death. Increased perioperative and postoperative opioid use during orthopedic shoulder surgery is a significant risk factor for long-term opioid dependence. The authors hypothesized that a multidisciplinary perioperative pain management program (Transitional Pain Service [TPS]) for major shoulder surgery would lead to a reduced amount of opioids required postoperatively.

  • A study was conducted to analyze obstetric outcomes in pregnant women with congenital uterine anomalies using data from the Nationwide Inpatient Sample between 2016 and 2019.* -
  • Out of 50,180 women assessed, the majority had a bicornuate uterus, with 70.6% achieving full-term live births, while 26.8% experienced preterm births.* -
  • The findings highlighted that women with uterine didelphys had the highest rates of preterm births, and 65.7% of all patients underwent cesarean sections, with increased risks of severe maternal morbidity associated with septate uteri.*

Study Question: What is the relationship between late follicular phase progesterone levels and clinic pregnancy and live birth rates in couples with unexplained infertility undergoing ovarian stimulation with IUI (OS-IUI)?

Summary Answer: Late follicular progesterone levels between 1.0 and <1.5 ng/ml were associated with higher live birth and clinical pregnancy rates while the outcomes in groups with higher progesterone levels did not differ appreciably from the <1.

Background: Opioid use disorder is a significant cause of morbidity, mortality, and health care costs. A transitional pain service (TPS) approach to perioperative pain management has been shown to reduce opioid use among patients undergoing orthopedic joint surgery. However, whether TPS also leads to lower health care use and costs is unknown.

Age-related decline in mobility and cognition are associated with cellular senescence and NAD + depletion in dogs and people. A combination of a novel NAD + precursor and senolytic, LY-D6/2, was examined in this randomized controlled trial. Seventy dogs with mild to moderate cognitive impairment were enrolled and allocated into placebo, low or full dose groups.
