Large-cell Neuroendocrine Carcinoma of the Endometrium in Myomatous Uterus.

Large-cell neuroendocrine carcinoma of the endometrium is an insufficiently researched aggressive tumor with a short survival regard-less of the treatment type.We present here the 38th consecutive case of literature reported cases of large cell neuroendocrine carcinoma of the endometrium (posi-tive for synaptophysin, CD56, chromogranin A, p53, and Vimentin) found in the myomatous uterus of a 76-year-old woman.We also describe in the study the morphological algorithm for differentiation of malignant blastomas with small (under 10 %) neuro-endocrinal component.

Aggressive juvenile mandibular fibromatosis.

Aggressive juvenile fibromatosis of the jawbones is a rare tumor presenting as infiltrative mass with unpredictable evolution. We report herein a 17-year-old student with a 6-month history of radiologically proven resorption of a part of the mandible, lingual displacement of tooth 34 and malocclusion. Alveolar ridge resorption and three dark-brown foci in the bone were seen after the tooth was extracted.

Cystic fibrosis and celiac disease--multifaceted and similar.

Celiac disease and cystic fibrosis share a number of clinical manifestations. The comorbidity rate of these diseases is low: 1:200000. We present a case of a child aged 1 year and 5 months, born to a mixed-marriage parents, with concomitant cystic fibrosis and celiac disease manifesting initially with chronic diarrhea.

[Necrotizing sarcoid granulomatosis--case report and review of the literature].

The Necrotizing sarcoid granulomatosis is a rarely met angiitis. The disease is predominantly localized in the lung and has a good prognosis. The authors reported about a patient treated in their clinic; the diagnosis is made after histologic examination.

Effect of mycophenolate mofetil (Cell Cept) in the treatment of immune glomerulopathies.

Aim: To study the effect of mycophenolate mofetil (Cell Cept) in treating patients with various types of chronic glomerulonephritis and other immune nephropathies.

Material And Methods: Between 2000 and 2003 we treated 35 patients (18 women, 17 men) with Cell Cept (La Roche). In 32 patients the diagnosis was confirmed by kidney biopsy (immunofluorescence, light and electron microscopy).

Wrist tuberculosis in cladribine--induced remission of hairy cell leukosis.

A 45-year-old man with "hand-shoulder" syndrome developing eight-months after Cladribine-induced remission of an 11-year-old hairy cell leukosis is presented. Wrist bone biopsy was performed because of failure of the algodystrophy treatment and radiographic findings of progressive osteoporosis. Caseating epiteloid granulomas abundant in Langhans cells were found histologically and later Mycobacterium tuberculosis species was isolated in culture specimen.

Primary malignant melanoma of oral cavity.

Primary oral malignant melanomas (POMM) are very rare. Definitive identification is usually possible only after they develop metastases. So every new case is instructive.

A denture complication: sarcoid-like foreign body granulomas of the tongue.

Sarcoidosis in the oral cavity occurs extremely rare (there have been described only 47 cases in the literature) and is almost always doubtful. We describe a protruding painful nodule (1 cm) on the apex of the tongue of a 65-year-old female, adjacent to the site of irritation of a new denture. Histologically it was confluent mass of non-caseous granulomas, rich in Langhans' cells.

[Peritoneal cyst--a case report].

Peritoneal cysts are rare benign tumors of the abdominal and pelvic peritoneum. We present a case of 26 year old patient with a peritoneal cyst treated operatively.

Giant fibroadenoma of the breast.

A case of giant fibroadenoma of the breast (size 23x20x13 cm and weight 2680 grams) in a 43-year-old woman with short stature and gracile body build is presented. Pericanalicular fibroadenoma was detected histologically. One year after mastectomy the patient is in good health, with no recurrences.

A case of undifferentiated (embryonal) sarcoma of the liver: fine needle aspiration cytology diagnosis by one cell type.

We present the first case in Bulgaria of cytologically verified undifferentiated (embryonal) sarcoma of the liver. A 10-year-old girl was admitted to the hospital with abdominal pain, extreme hepatomegaly, one-month fever, anemia and highly elevated erythrocyte sedimentation rate (95 mm). A large formation was found in the right liver lobe on ultrasound examination, which was polycystic on CT scan.

Primary localized amyloidosis of the urinary bladder. A case report.

We present case 96 (the first one in Bulgaria) of primary localized amyloidosis of the urinary bladder in a 72-year-old woman with episodes of painless hematuria. The amyloid tumor is situated on the anterior wall of the bladder--a yellowish prominence 15 mm in diameter with superficial erosion that is surrounded by a reddish ring and bullous edema of the mucosa. Rough and regularly pink perivascular deposits, thickened vessel walls and focal lymphoplasmocyte infiltrates are present in the submucosa.

Cyclosporin A in the treatment of patients with nephrotic syndrome.

Objective: To present our experience in the treatment of conventional therapy refractory nephrotic syndrome with cyclosporin A.

Material And Methods: The study sample included 22 patients (12 men, 10 women, aged 40.43 +/- 5.

Treatment of immune nephropathies with high doses of immunoglobulins.

Unlabelled: The present study presents the results from the application of high doses of gamma-globulin in the treatment of immune (idiopathic and satellite) glomerulopathies.

Materials And Methods: Twenty patients were treated. Of these 12 were with primary chronic glomerulonephritis, 7--with lupus nephritis and 1--with renal amyloidosis.

[Views of pathologists and clinicians on autopsy].

The study was performed using questionnaires which were filled in by 114 pathologists and 210 clinicians. The pathologists considered the major attractive professional motivations to be the elucidation of clinical aspects and possibility to be engaged in biopsy diagnosis, the most unattractive thing was a small salary. The clinicians supported the view that an autopsy was obligatory, the ratio of supporters to ++nonsupporters among clinicians and pathologists being 2.

[Ultrastructural study of the interstitial cells of the internal kidney medulla exposed to indomethacin].

Unusual cylindrical bodies-megatubules, which get in touch with lipid granules with granular endoplasmatic reticulum among themselves and with the pores of the nuclear and cellular membrane, are described in the interstitial cells of the kidneys of the control rats. After a suppression of prostaglandin synthesis with indomethacin (5 mg/kg venously of intraperitonealy per day for 6 days) there was statistically significant increase of the mass of the compact lipids in the interstitial cells (hypergranulation as the volume of the granule was preserved) - a proof that the granules contained precursors of renal prostaglandins. The nuclei were presented with round contours, while the perinuclear spaces were narrowed.

[Structural organization of the tubular system of the internal renal medulla interstitial cells].

The tubular system of renal medulla interstitial cells includes multitubular complexes, mucrotubules and megatubules. Multitubular complexes are compact masses consisting of dozens of crosscut thin-walled tubular lumens. Megatubules are 2500 A in diameter, up to 5.

Ultrastructural investigation of renomedullary interstitial cells in "endocrine kidney".

In the renomedullary interstitial cells of rats with Selye's "endocrine kidney" in the phase of beginning hypertension there appeared bilateral stress hypergranulation and an ultrastructural pattern of intensive prostaglandin synthesis. In the interstitial tissue of the left medulla there appeared light and dense (fibroblast- or macrophage-like) cells. Along with the stabilization of hypertension a relative degranulation of interstitial cells in the right kidney and an absolute degranulation in the left kidney took place.
