Especially in postmenopausal women, the clinical manifestation of a mature teratoma with undiagnosed malignant transformation as acute abdomen is extremely rare. In these cases, total hysterectomy with bilateral salpingo-oophorectomy is the treatment choice since the chance of malignancy is high.
