We describe the case of a 2-year-old girl showing a pedunculated lesion on the areola of the nipple since birth. Clinical and histological features were consistent with an adnexal polyp of neonatal skin observed beyond the neonatal period.

Nail involvement preceded the identification of osteolytic lesions at the mastoid on a cranial computed tomography scan, which was performed because of repeated episodes of acute otitis media. Some weeks later, a small number of erythematous papules developed over the trunk and face.
