Background & Aim: Sternotomy remains a commonly used technique to access the heart for cardiac surgery worldwide. To date, there is no clear consensus on the single superior sternal closure technique. Patient-specific factors such as osteoporosis, diabetes, old age, body habitus influence a surgeon's choice in this matter as do techniques commonly used during the training period and used in the current workplace.

Article Synopsis
  • The study compares angiographic patency among three types of left internal mammary artery (LIMA) graft configurations: single, sequential, and Y grafts.
  • Results showed that single grafts had the highest patency rates, with 3.7% failure in segments, compared to sequential (9%) and Y grafts (6.2%).
  • Female patients and those with sequential grafts have lower patency rates for the initial segments of the LIMA, suggesting a preference for single grafts when possible, while Y grafts may be useful when arterial options are limited.

Article Synopsis
  • The Ross procedure is a heart surgery that is usually not done on older patients, but this study looks at how older patients (50 and up) do compared to younger patients after the surgery.
  • The study followed 96 patients from both age groups and found that both had similar chances of surviving for 15 years after the surgery.
  • It turns out that older patients had good results, with very few needing additional surgeries, so healthy older patients should also be considered for this operation.

Narrow mitral annulus is a common situation faced in adult patients. Choice of prosthetic valve in such cases is unclear. There are reported experiences with supra-annular mitral valve, Manouguian technique, "chimney technique" with mechanical valve and Melody valve, the latter two being in children.
