Background: The utility of Next-Generation-Sequencing (NGS) in patients of non-small-cell-lung-cancer (NSCLC) has led to an exponential increase in the identification of driver-gene alterations, however, Indian NGS data was lacking.

Materials And Methods: This retrospective study conducted between May'2019 and Dec'2023 included histologically confirmed NSCLC cases with NGS testing done on tissue and/or liquid biopsy samples prior to treatment initiation. We reported the frequency of driver-gene alterations, clinicopathological profile, treatment patterns, and outcomes [Overall-Survival (OS)].

Article Synopsis
  • ALC-positive lung cancers generally respond well to oral tyrosine kinase inhibitors, with Lorlatinib being a preferred option after previous treatments; this study examines its use in patients who progressed or couldn't tolerate earlier therapies.
  • In a study of 38 patients treated with Lorlatinib, most had extensive metastatic disease (mainly to the brain and bones), with a median follow-up of 49 months, revealing that 84% experienced disease control.
  • Results indicated a median progression-free survival of 16 months and overall survival of 22 months, with common side effects including elevated cholesterol and triglycerides; some patients needed dose adjustments due to these toxicities.

Background: Standard neoadjuvant chemotherapy for locally advanced esophageal or gastroesophageal junction squamous cancer, 5-fluorouracil plus platinum, is toxic and logistically challenging; alternative regimens are needed.

Methods: This was a phase III randomized open-label noninferiority trial at Tata Memorial Center, India, in resectable locally advanced esophageal or gastroesophageal junction squamous cancer. Patients were randomly assigned 1:1 to 3 cycles of 3-weekly platinum (cisplatin 75 mg/m2 or carboplatin area under the curve 6) with paclitaxel 175 mg/m2 (day 1) or 5-fluorouracil 1000 mg/m2 continuous infusion (days 1-4), followed by surgery.

Article Synopsis
  • No definitive studies previously showed that chemotherapy improved overall survival in advanced esophageal cancer, prompting a Phase III randomized trial comparing best supportive care (BSC) alone to BSC plus weekly paclitaxel.
  • The trial involved 281 patients, revealing that those on chemotherapy experienced a median overall survival of 9.2 months, compared to only 4.2 months for those on BSC.
  • Results showed that chemotherapy significantly improved response rates, progression-free survival, and quality of life, while not increasing severe toxicities, marking it as a valuable treatment option in this population.

Introduction: Osimertinib is more efficacious and as safe as first-generation epidermal growth factor receptor (EGFR)-directed tyrosine kinase inhibitors. However, osimertinib is not affordable for most patients in developing nations. Moreover, the minimum biologically effective dose of osimertinib may be less than the approved dose.

Article Synopsis
  • The study investigates the imaging characteristics of supratentorial ependymomas (STE), rare tumors primarily found in children, using CT and MRI scans from 25 patients.
  • Key findings suggest that STEs often occur near ventricles and are associated with cystic components and specific imaging signs like the periwinkle sign.
  • The research also highlights significant links between these imaging features and the presence of a particular genetic alteration (ZFTA fusion), aiming to improve diagnosis and treatment outcomes.

Kimura disease (KD) is a rare chronic inflammatory disorder characterized by the development of painless subcutaneous nodules, predominantly in the head and neck region. Diagnosis relies on a high index of clinical suspicion and clinicopathological correlation, with core biopsy serving as the gold standard for a definitive diagnosis. While the disease itself is benign, it can cause significant morbidity if left untreated.

Purpose: Febrile neutropenia (FN) is a serious complication in hematologic malignancies, and lung infiltrates (LIs) remain a significant concern. An accurate microbiological diagnosis is crucial but difficult to establish. To address this, we analyzed the utility of a standardized method for performing bronchoalveolar lavage (BAL) along with a two-step strategy for the analysis of BAL fluid.

Introduction: The outcomes in advanced NSCLC have improved owing to the availability of more effective systemic and improved supportive care. This has increased the number of patients who seek treatment in the third line and beyond setting. We conducted this study to compare the quality of life (QoL), toxicity, and outcomes in patients receiving chemotherapy and EGFR tyrosine kinase inhibitors (TKIs) in this setting.

is an aerobic, nonfermentative Gram-negative bacillus infrequently isolated from clinical specimens. The pathogenicity and clinical significance of the organism has not been ascertained due to uncommon clinical isolation and suspected low virulence. The organism has been reported to be inherently resistant to aminoglycoside group of drugs which remain as a widely used first-line drug of choice for febrile neutropenic patients.

Lung cancer is the most diagnosed cancer worldwide. Many non-malignant pulmonary lesions, such as tuberculosis, fungal infection, organizing pneumonia, inflammatory myofibroblastic tumor, and IgG4 disease, can mimic lung cancer due to their overlapping morphological appearance on imaging. These benign entities with minor differentiating imaging clues may go unnoticed in a high-volume cancer institution, leading to over-investigation that may result in repeated biopsies, pointless wedge resections, and related morbidities.
