Aim: Volar locking plates designed for far distal radius fracture fixation can have a significant hardware removal rate and risk of tendon rupture. Plate design has a role in the rate of complications. This study assessed the hardware removal and tendon rupture rate of the Acu-Loc 2 volar distal radius (VDR) plate often used in the treatment of far distal radial fractures.

A Face-to-Face Surgical Instrumentation Course During the COVID-19 Pandemic.

Objectives Surgical instrumentation teaching is included as an essential part of surgical training in the core surgical training syllabus. Access to formal teaching is variable, and opportunities for informal teaching have been further reduced by the COVID-19 pandemic. We aimed to design a course to fulfil these local trainees' needs.

Capitate shortening osteotomy and vascularized bone grafting for Kienböck's disease in ulnar positive or neutral wrists.

We report the short- to medium-term outcomes for patients with Kienböck's disease and ulnar positive or neutral wrists treated by capitate shortening osteotomy combined with a 4 + 5 extensor compartmental artery vascularized bone graft placed in the lunate. This is a retrospective study of seven consecutive patients with Lichtman Stage 2 to 3B. Radiological and clinical outcomes were evaluated.

The use of low-intensity pulsed ultrasound in hand and wrist nonunions.

The management of hand and wrist nonunions is challenging and alternatives or adjuncts to surgery to promote healing are an attractive prospect. Low-intensity pulsed ultrasound (LIPUS) is reported to improve bone healing and is supported for use in nonunions. However, evidence supporting its use for established nonunions is based largely on long bones, with little evidence guiding use in the hand and wrist.

Candida parapsilosis osteomyelitis.

A 51-year-old previously fit and healthy gentleman sustained a circular saw injury to his right thumb with partial amputation and an open multifragmentary fracture of his distal phalanx. He underwent open reduction and internal fixation under the hand surgery team. He developed a postoperative infection discharging pus 2 weeks postoperatively, which later grew Candida parapsilosis.

Procedure-based assessments in trauma and orthopaedic training--The trainees' perspective.

Introduction: The study aimed to gain an understanding of the attitudes of trauma and orthopaedic (T&O) trainees regarding procedure-based assessments (PBAs) and identify factors that influence any perceived educational benefit.

Methods And Materials: A questionnaire was emailed to all T&O trainees in the UK via an established e-mail communication tool after an initial pilot exercise. The data were analysed using the online survey software.

A radiological sign in chronic collateral ligament injuries of the thumb metacarpophalangeal joint.

Background: Differentiating chronic from acute injuries of the collateral ligament of the metacarpophalangeal joint (MCPJ) of the thumb can be difficult in the absence of a conclusive history and examination. We aimed to establish the presence of a radiological sign in patients with a chronic injury and the reliability of the sign in differentiating chronic from acute ligament injury.

Methods: Consecutive patients undergoing surgical treatment for chronic (n = 14) or acute (n = 8) ligament injury of the MCPJ of the thumb were identified retrospectively.

Bilateral thumb metacarpophalangeal joint fusions for severe hyperextension deformities in conjunction with carpometacarpal joint reconstructions.

Background: Hyperextension of the first metacarpophalangeal (MCP) joint and adduction of the first web space of the hand are features of advanced stages of carpometacarpal (CMC) joint arthrosis. Restoration of mechanical efficiency in such patients requires stabilization of both the CMC and MCP joints. This study describes a patient with bilateral secondary hyperextension of the MCP joint greater than 50° who displayed good functional outcome following staged bilateral MCP joint arthrodesis with simultaneous CMC joint reconstruction.
