It has been well established that the human gut microbiota plays a pivotal role in humans' health, since it is involved in nutrients' uptake, vitamins' synthesis, energy harvest, inflammatory modulation, and host immune responses. Moreover, gut microbiota alterations have been associated to an increasing number of diseases and its composition can be affected by several factors, including physical exercise. In particular, it has been reported that intense physical activity can induce metabolic changes which translate in alterations of specific biomarkers that can lead to the onset of infections, inflammation and hepatic or kidney disorders.

Article Synopsis
  • - The introduction of ETI therapy has improved life expectancy for adults with cystic fibrosis, but it raises concerns about long-term health issues, particularly cardiovascular risks due to existing factors linked to CF.
  • - A study involving 58 adults with CF assessed changes in cardiac and metabolic biomarkers after 6 months on ETI therapy, revealing increased cholesterol and triglycerides levels while inflammatory markers decreased.
  • - The findings indicate a disruption in hormone regulation related to obesity and cardiovascular risk, suggesting that while ETI improves some aspects of health, it negatively affects lipid profiles in patients, leading to potential metabolic issues.

  • The introduction of modulators, correctors, and potentiators for the CFTR molecule has significantly improved the health of most individuals with cystic fibrosis (CF).
  • While these therapies impact certain fatty acids, they do not address linoleic acid (LA) deficiency, which is linked to more severe forms of CF.
  • The review examines lipid abnormalities in CF patients, such as altered phospholipid and cholesterol levels, and their connection to clinical symptoms, revealing these lipid issues can manifest before birth.

  • The study examines the effectiveness of elexacaftor/tezacaftor/ivacaftor (ETI) therapy in improving lung function in people with cystic fibrosis (pwCF), specifically looking at the lung clearance index (LCI) as a sensitive measure.
  • 129 pwCF, aged 12-36, were assessed at the start of ETI and after 6 and 12 months, revealing that while most showed significant improvement in lung function measurements (FEV), a small group did not (15.5% had no significant change).
  • Those with minimal change in FEV had better initial pulmonary function, yet showed improved LCI values post-treatment, highlighting the LCI's role in evaluating ETI effectiveness

Article Synopsis
  • The study examined the link between dietary patterns and gastric adenocarcinoma (GA) in Brazil, focusing on how certain nutrients affect this relationship.
  • Researchers identified two main dietary patterns: "unhealthy dietary pattern" (UDP), which increased GA risk, and "healthy dietary pattern" (HDP), which decreased it.
  • The findings revealed that added sugars and sodium intake played significant roles in mediating the effects of these dietary patterns on GA risk, providing insights for future preventive public health strategies.

Cancer burden can be reduced by controlling modifiable risk factors, including diet. We provided an evidence-based assessment of cancer cases and deaths attributable to diet in Italy in 2020. We considered dietary factor-cancer type pairs for which the World Cancer Research Fund/American Institute for Cancer Research - Continuous Update Project reported either 'convincing' or 'probable' evidence of causal association.

Background: Bronchiectasis is a chronic respiratory condition characterised by airway and systemic inflammation with prevalence increasing with age. Given the median age of the patients, it is common to observe the presence of comorbidities, particularly cardiovascular diseases, which have been linked to adverse clinical outcomes. To investigate the pooled estimates of the association between bronchiectasis and coronary heart disease or stroke within this population, we conducted a systematic review and meta-analysis of the available scientific evidence.

The introduction of CFTR modulator drugs like elexacaftor-tezacaftor-ivacaftor (ETI) has transformed the management of cystic fibrosis (CF), significantly improving symptoms, lung function, and quality of life, while reducing reliance on intravenous antibiotics. However, respiratory exacerbations in the CFTR modulators era remain poorly understood from both pathophysiological and clinical perspectives. We present the case of a 20-year-old Caucasian woman with CF (F508del/L1077P) who, after three years of ETI treatment, experienced a severe episode of hemoptysis, despite being almost asymptomatic in the weeks leading up to admission, requiring bronchial artery embolization.

This study assessed humoral and T cell-mediated immune responses to the BNT162b2 vaccine in orthotopic liver transplant (OLT) and lung transplant (LUT) recipients who received three doses of the vaccine from March 2021 at our institution. Serum samples were collected 60 days post-second and third dose to quantify antibodies against the spike region of SARS-CoV-2 while whole blood samples were collected to analyze the SARS-CoV-2-specific T-cell response using an IFN-γ ELISpot assay. We enrolled 244 OLT and 120 LUT recipients.

Background: Elexacaftor-tezacaftor-ivacaftor (ETI) is a highly effective cystic fibrosis (CF) transmembrane conductance regulator (CFTR) modulating therapy for people with CF and at least one F508del variant. However, there is limited data about the safety and efficacy of this therapy in pediatric populations and in real-world settings. This study aimed at evaluating the effectiveness, tolerability, and safety of ETI in children with CF.

Background: Highly effective modulators of the CFTR channel have been demonstrated to dramatically impact disease progression and outcome. However, real-world data indicates that the magnitude of the clinical benefit is not equal among all patients receiving the treatment. We aimed to assess the variability in treatment response (as defined by the 6-month change in sweat chloride concentration, forced expiratory volume in one second [ppFEV1], body mass index [BMI], and CF Questionnaire-Revised [CFQ-R] respiratory domain score) and identify potential predictors in a group of patients receiving Elexacaftor-Tezacaftor-Ivacaftor (ETI) triple combination therapy.

Background: The effects of elexacaftor/tezacaftor/ivacaftor (ETI) on respiratory outcomes for people with cystic fibrosis (CF) were demonstrated by several clinical trials, mainly based on simple spirometry. However, gains in lung function may vary greatly between patients, and predictors of FEV change after treatment have yet to be defined.

Research Question: Which ventilatory parameters are involved in the heterogeneity of FEV change after 12-month ETI treatment in people with CF and advanced lung disease?

Study Design And Methods: This was a multicenter, observational, prospective cohort study at two major CF centers in Italy.

Evidence on the relationship between legume consumption and risk of specific cancer sites is inconclusive. We used data from a series of case-controls studies, conducted in Italy and in the Swiss Canton of Vaud between 1991 and 2009 to quantify the association between legume consumption and several cancer sites including oral cavity, esophagus, larynx, stomach, colorectum, breast, endometrium, ovary, prostate and kidney. Multiple logistic regression models controlled for sex, age, education, smoking, alcohol, body mass index, physical activity, comorbidities, and consumption of fruit, vegetables, processed meat and total calorie intake were used to estimate the odds ratios (OR) for different cancer sites and their corresponding 95% confidence intervals(CI).

Introduction: Over the last ten years an increasing prevalence and incidence of non-tuberculous mycobacteria (NTM) has been reported among patients with cystic fibrosis (CF) Viviani (J Cyst Fibros, 15(5):619-623, 2016). NTM pulmonary disease has been associated with negative clinical outcomes and often requires pharmacological treatment. Although specific guidelines help clinicians in the process of diagnosis and clinical management, the focus on the multidimensional assessment of concomitant problems is still scarce.
