Thirty-five patients with community-acquired pneumonia were examined. Studies of red blood cells and expired air condensate revealed significant nitric oxide metabolic disturbances in them. In a group of 17 patients, the use of N-acetylcysteine in the complex therapy resulted in the normalization of most parameters that characterized nitric oxide metabolism (nitrates, nitrites, peroxynitrite, NADP-H-diaphorase, and nitrate reductase activity).

The effects of nitric oxide on the development of pulmonary hypertension were studied in patients with bronchial asthma. The degree of pulmonary hypertension and the status of the left and right ventricles were determined by echocardiography. A significant increase in the nitric constituent was found in patients with moderate bronchial asthma, during is exacerbation in particular.

The diagnosis of viable myocardium in patients with ischemic heart disease (IHD) with left ventricular (LV) dysfunction is essential for prediction of effectiveness and validity of myocardial revascularization as well as of prognosis in such patients. Great advances in intravascular reconstructive interventions on coronary arteries makes this diagnosis still more important. For diagnosis of reversible forms of LV myocardial dysfunction (stunned and hibernating myocardium) such highly informative methods as positron-emission tomography, single-photon emission computed tomography, nuclear magnetic resonance may be applied but they are not easily available because of high cost.

The results of the authors' investigations demonstrate that sandimmun in mean therapeutic doses given to patients with chronic glomerulonephritis produces nephrotoxic complications in rare cases even when nephroprotective vasodilators are not used. However, administration of the latter (nifedipin, tenziamin, etc.) improves renal hemodynamics that is increases effective renal plasm and blood flow to normal values, lowers renal vascular resistance after 2-3 months of therapy.

The indices of local nonspecific lung protection in the bronchoalveolar lavage fluid and cells were studied in 20 patients with acute pneumonia and 20 patients with lingering pneumonia. In acute pneumonia, the cellular and humoral links of local protection were found to have an adequate responsiveness and to preserve their protective properties during conventional therapy. In lingering pneumonia, the phagocytic and microbicidal properties of alveolar macrophages and the levels of lysozyme and sIgA in the bronchoalveolar lavage were lower.

A study of platelet aggregation and platelet sensitivity to prostacyclin in 82 patients with acute myocardial infarction demonstrated a relationship between clinical pattern of the disease and platelet hemostasis. Platelet functional activity was the highest by the end of week 2 in uncomplicated myocardial infarction. In cases where myocardial infarction was complicated by acute left-ventricular insufficiency, platelet function differed depending on the severity and outcome of the complication.
