Posteromedial dislocation of the elbow is a rare entity even exceptional if associated with a lateral condylar fracture. Very few isolated cases are reported in the English literature. We report an additional case of a 14-year-old child admitted for elbow trauma during scholar sport and presented a posteromedial elbow dislocation associated with a fracture of the lateral humeral condyle.

Herein, we report the case of a 39-year-old man who presented with pain and total functional impotence in his right elbow following a fall; the clinical and radiological findings revealed a new entity in the coronal shear fractures of the distal humerus, which included a comminuted fracture of the capitellar and displaced separate trochlea in the same elbow. This fracture was treated surgically with a good outcome at the last follow-up.

A closed reduction was performed without complications, and one month later the right shoulder was stabilized by open Laterjet technique. The evolution was favorable for both shoulders with no pain, recuperation of the range motion and no recurrence of dislocation.
