Article Synopsis
  • This study assesses the cost-effectiveness of mobile stroke units (MSUs) compared to standard emergency medical services (EMS) for administering thrombolytics in ischemic stroke patients.
  • The analysis used the incremental cost-effectiveness ratio (ICER) method to evaluate healthcare costs and quality-adjusted life years (QALYs), highlighting considerable variations based on patient disability status and treatment costs.
  • Overall, while MSUs show borderline cost-effectiveness for all eligible patients, they are more favorable for patients without pre-existing disabilities, particularly when focusing on lifetime costs and stroke-specific expenses.

Article Synopsis
  • Yeast cells maintain low cytosolic calcium levels using transporters, with the vacuolar Ca ATPase Pmc1 playing a key role in this process.
  • Inactivation of Pmc1 leads to hypersensitivity to sodium dodecyl sulfate (SDS), which can be counteracted by inactivating the high-affinity calcium channel Cch1.
  • Low concentrations of SDS cause a quick influx of external calcium into the cells, but the mechanism of Cch1's involvement suggests it has additional functions unrelated to its role in calcium transport.

  • Alzheimer's disease is associated with impaired proteostasis, and enhancing it might help delay cognitive decline.
  • KPT-330 (Selinexor), an XPO1 inhibitor, shows promise in improving proteostasis and enhancing autophagy in neurons, leading to better memory in Alzheimer's models.
  • While KPT-330 increased overall amyloid deposition in the brain, it also resulted in lower levels of amyloid in specific areas like the thalamus, indicating that its effects on proteostasis and amyloid formation are region-specific.

  • Researchers developed a novel single-cell method called "Mirror" that allows for the study of nuclear non-coding RNAs by visualizing their pathways through cytoplasmic fluorescence.
  • This technique successfully identified key components involved in the maturation and degradation of the long non-coding RNA MALAT1, as well as other RNA complexes critical for RNA processing.
  • Additionally, the study found that the DEAD-box helicase DDX59 is associated with Oral-Facial-Digital syndrome and plays a role in stabilizing MALAT1 RNA, suggesting important links between these pathways and human diseases.

  • GLP-1 receptor agonists (liraglutide, semaglutide, tirzepatide) may lower cardiovascular risk in overweight/obese individuals without diabetes.
  • A systematic review of 16 trials involving over 28,000 participants showed these medications reduced major adverse cardiovascular events and all-cause mortality compared to placebo.
  • While they significantly lowered the risk of myocardial infarction, the relationship with stroke is less clear due to limited data.

Right-to-left shunt, mainly due to patent foramen ovale (PFO), is likely responsible for ≈5% of all ischemic strokes and 10% of those occurring in young and middle-aged adults. Randomized clinical trials demonstrated that, in selected young and middle-aged patients with otherwise cryptogenic acute ischemic stroke and high-risk PFO, percutaneous PFO closure is more effective than antiplatelet therapy alone in preventing recurrence. However, PFO is generally a benign finding and is present in about one-quarter of the population.

Herein, we propose a blueprint for action to completely measure and recognize the care provided by acute and critical care nurses to be incorporated into policy that shapes and supports practice. We address the nature of nurses' work by identifying nine practice domains, hospital practice environment assumptions, and expected outcomes. Nurses' work, as a cross-system process, needs to be included in hospital-based core measures to fully reflect nurses' impact on patient care.

Recently, progress has been made in fabricating diamond-based scintillators with integrated rare-earth luminescent particles. These luminescent particles are integrated into the bulk of diamond during their synthesis by chemical vapor deposition (CVD). However, the growth conditions include a chemically aggressive plasma environment and elevated temperatures, which results in the partial degradation of particles and a decrease in the intensity of their luminescence.

In the assessment and management of many clinical problems, point-of-care (PoC) ultrasound is an emerging bedside tool. Transcranial color-coded duplex (TCCD) ultrasound can be valuable in multiple situations, including for patients who are unconscious or have an equivocal neurologic examination, as it helps rule in specific intracranial pathologies. Despite the known diagnostic value of transcranial ultrasound, its use in critical care medicine remains variable.

Autoerotic death, as a subtype of mechanical asphyxia, refers to a person's fatal outcome while engaging in solitary sexual activity using various devices and methods to reduce oxygen supply and induce cerebral hypoxia, leading to increased sexual gratification. These asphyxial deaths are accidental and sporadic. In cases of sexual asphyxia, especially when strangulation methods such as hanging or ligature are used, thorough crime scene investigation is crucial to determine the type of asphyxia and the manner of death.

Article Synopsis
  • - The study aimed to compare levels of cerebrospinal fluid (CSF) and plasma biomarkers associated with Cerebral Amyloid Angiopathy (CAA) against healthy controls and Alzheimer’s Disease patients.
  • - A meta-analysis of nine cohorts revealed significant differences in CSF levels of β-amyloids and tau proteins, with CAA patients showing lower Aβ42 and Aβ40 levels compared to healthy controls and distinct tau levels compared to both groups.
  • - The findings suggest that specific biomarker profiles in CSF could help differentiate CAA from Alzheimer's and healthy individuals, while plasma biomarker levels did not show significant differences.
