Cholestasis has numerous causes. We present the case of a 78-year-old man with a common diagnosis in this age group and gender but with an unusual presentation. There are only 11 articles published of patients with jaundice due to a paraneoplastic syndrome associated with prostate cancer.

The hyponatraemia study was compatible with syndrome of inappropriate antidiuresis (SIAD) and an association with the respiratory problem was initially assumed. The recurrence of hyponatraemia after resolution of the pulmonary infection led to further investigation and to the diagnosis of tuberculous lymphadenitis.

He had a meatal stenosis and had a suprapubic catheter for 10 months, and had a recent hospitalisation for urosepsis with bacteraemia due to methicillin-resistant Staphylococcus aureus after manipulation of the catheter. Clinical examinations were performed in the emergency department and the patient was hospitalised with the diagnosis of recurrent urinary tract infection.
