Treatment of functioning insulinoma using endoscopic ultrasound-guided radiofrequency ablation.

85-year-old patient who underwent endoscopic ultrasound (EUS)-guided radiofrequency ablation for a functioning pancreatic insulinoma and was previously treated with acenocoumarol due to a high thrombotic risk. Acenocoumarol was discontinued three days prior to the procedure and replaced with enoxaparin at 40 mg daily, with the basal anticoagulant therapy being restarted 24 hours after the procedure. Ten days later, the patient was diagnosed with an acute hematoma in the tail of the pancreas and gastric body, which led to clinical signs of hypotension and acute anemia, requiring the discontinuation of anticoagulation once again.

Duodenal perforation due to biliary plastic stent. Endoscopic management.

We present the case of a patient who presents a duodenal perforation after placement of a plastic biliary stent due to pancreatic head neoplasia, who underwent endoscopic removal of the biliary stent and closure of the perforation with OTSC.

Cannabis intake and intussusception: an accidental association?

Most intussusception cases in adults have an organic cause and their treatment is surgical. In some cases, there is no injury associated with intussusception and we can opt for conservative management. In our clinical practice we have shown the presence of intussusceptions in the absence of structural damage associated with chronic cannabis with a good course after conservative management.

Hemospray treatment is effective for lower gastrointestinal bleeding.

Acute lower gastrointestinal bleeding (LGIB) is diverse in origin and can be substantial, requiring urgent hemostasis. Hemospray is a promising novel hemostatic agent for upper gastrointestinal bleeding (UGIB). It has been claimed in a small series that the use of Hemospray is also feasible in LGIB.

Triptans are a class of drugs with proven efficacy in the acute treatment of migraine headache. The first component of these drugs was sumatriptan, with various derivatives subsequently emerging. Until now, there has only been one reported case of liver toxicity with zolmitriptan.
