Article Synopsis
  • A study aimed to compare the prevalence of obsessive-compulsive symptoms (OCS) among schizophrenia patients on different antipsychotics (clozapine, olanzapine, risperidone) and those not taking any medication.
  • Results showed that 23.4% of patients with schizophrenia experienced OCS, significantly higher than the 4.9% in healthy controls, with clozapine users reporting the highest rates (38.9%).
  • The findings suggest that prolonged clozapine treatment (6 months or more) is linked to an increased prevalence of OCS, while risperidone and olanzapine users, along with those not on medication, had lower yet comparable OCS rates

Substance abuse and psychotic disorders have a high rate of comorbidity. Both disorders are associated with changes in the dopaminergic transmission in the mesocorticolimbic pathways of the brain. Since antipsychotic medications interact with the dopamine receptors in these pathways, these medications could affect craving for substances.

Impulse regulation disorders and substance abuse disorders have considerable consequences for treatment and prognosis of comorbid psychiatric disorders. Second generation antipsychotics (SGA) are frequently prescribed off-label for these disorders. This off label use of SGA entails some systematic problems, such as lack of knowledge about their long-term efficacy and effects, including side-effects, and unclarity about doses in certain disorders and patient groups, for example children and adolescents.
