Delayed progressive haematoma after Le Fort I osteotomy: A possible severe complication in orthognatic surgery.

Although the Le Fort I osteotomy is a safe surgical technique, many complications have been reported. We present a case of an extended cervico-facial haematoma due to delayed bleeding from the terminal branches of the maxillary artery after orthognatic surgery. A 23-year-old man was referred to our observation for the surgical correction of a class III asymmetric malocclusion.

Bilateral pneumothorax after orthognatic surgery.

Among complications in orthognathic surgery, the insurgence of pneumothorax is very rare. Pneumothorax is the presence of air or gas in the pleural cavity and it is rare complications in the postoperative oral and maxillofacial surgery patient. The clinical results are dependent on the degree of collapse of the lung on the affected side.

STABLE: an innovative device for three-part maxillary osteotomy.

Le Fort I osteotomy of the maxilla continues to be one of the most common techniques used in the surgical correction of a variety of dento-midfacial deformities. Occasionaly, however, surgeons may encounter difficulties during three-pieces Le Fort I procedures because the surgical movements are also prone to adverse movement and subsequent relapse. This case report describes a 26 year old man, who presented with a skeletal Class III malocclusion and a transverse maxillary deficency.

Full vestibular approach in pre-prosthetic surgery: retrospective evaluation of 90 patients and literature review.

Atrophy of the jaws could represent a challenge in maxillofacial and oral surgery. The correction of bone defects due to the atrophic process is frequently achieved by means of onlay bone grafting. In the present note, a technique of flap designing to approach the alveolar atrophic crest will be discussed.

Background: Prescribing benzodiazepines during buprenorphine treatment is a topic of active discussion. Clinical benefit is unclear. Overdose, accidental injury, and benzodiazepine misuse remain concerns.

Electroencephalography and dementia: a literature review and future perspectives.

While many studies have investigated electroencephalographic (EEG) features of dementia, few have analysed the relationship between EEG and cerebrospinal fluid biomarkers in cognitive impairment. Seizures are frequently observed at the end stage of Alzheimer disease, and experimental animal studies support the view that epileptiform activity may contribute to the cognitive decline. In this paper, after reviewing literature findings concerning the role of EEG in dementia, we show the preliminary results of our study aimed to correlate the presence of epileptiform EEG patterns with cerebrospinal fluid biomarkers in order to better define the prognosis of dementia.

Combined rhinoplasty and genioplasty: long-term follow-up.

Increased cortical excitability after selective REM sleep deprivation in healthy humans: a transcranial magnetic stimulation study.

Background: REM sleep has antiepileptogenic properties whereas, its loss is known to have a proconvulsive role. However, the mechanisms underlying the proepileptogenic effects of REM sleep deprivation are yet not fully understood. The aim of our study was to evaluate the effects of selective REM sleep deprivation (SRD) on cortical excitability in healthy subjects by means of transcranial magnetic stimulation (TMS).

Application of the "All-on-Four" concept and guided surgery in a mandible treated with a free vascularized fibula flap.

In this article, we describe the first case in the literature in which 3D computer-assisted treatment planning and guided surgery enabled a patient affected by extreme paraphysiologic mandibular bone atrophy to be treated with a free vascularized fibula flap and, after a period of healing, the flapless installation of 4 immediately loaded dental implants. The computer-fabricated surgical guide allowed placement of the implants according to the "All-on-Four" concept in a proper spatial preplanned position and by tilting 1 implant, avoiding an area of bone discontinuity. Additionally, this technique allowed the production of a prefabricated temporary prosthesis, delivered after implant insertion, which could be immediately loaded.

Sliding genioplasty using fresh-frozen bone allografts.

OBJECTIVE To present our experience in the use of fresh-frozen human bone allograft as an interpositional grafting material for sliding genioplasty to correct chin deformities. METHODS Ten patients underwent sliding genioplasty using morcellized and corticospongious fresh-frozen human bone. Four patients underwent orthognathic surgery associated with genioplasty.

Spasticity as an ictal pattern due to excitotoxic upper motor neuron damage.

We describe the case of a man who presented with spasticity and aphasia related to continuous electroencephalographic epileptic activity in the left frontal-temporal regions. Magnetic resonance imaging (MRI) documented in diffusion-weighted images (DWI) two areas of restricted diffusion in the left frontal and temporal cortex. After starting treatment with levetiracetam 3000 mg/day there was progressive recovery of the clinical picture as well as the gradual disappearance of the electroencephalographic seizure activity and the vanishing of areas of restricted diffusion in brain MRI.

Psychiatry in the Harvard Medical School-Cambridge Integrated Clerkship: an innovative, year-long program.

Objective: The authors present what is to their knowledge the first description of a model for longitudinal third-year medical student psychiatry education.

Method: A longitudinal, integrated psychiatric curriculum was developed, implemented, and sustained within the Harvard Medical School-Cambridge Integrated Clerkship. Curriculum elements include longitudinal mentoring by attending physicians in an outpatient psychiatry clinic, exposure to the major psychotherapies, psychopharmacology training, acute psychiatry "immersion" experiences, and a variety of clinical and didactic teaching sessions.

Grafting of large mandibular advancement with a collagen-coated bovine bone (Bio-Oss Collagen) in orthognathic surgery.

Current principles for correction of dentoskeletal deformities ask to satisfy different treatment goals, making large mandibular advancements a common practice in orthognathic surgery. A main consequence of significant mandibular movements is the potential for unfavorable bone healing of osteotomy sites after traditional sagittal split procedures. This drawback, which mainly occurs at the level of inferior mandibular borders, can affect the stability and support of overlying soft tissues.

Mandibular reconstruction using fresh frozen bone allograft after conservative enucleation of a mandibular odontogenic myxoma.

The purpose of this article was to report the clinical, radiographic, and histological findings about a case of a young woman affected by a mandibular odontogenic myxoma. Conservative tumor resection was followed by immediate reconstructive treatment using fresh-frozen human bone graft, instead of autologous bone graft, as material for bone regeneration. Odontogenic myxoma, according to the World Health Organization, is classified as a benign tumor of mesenchymal origin whether or not containing odontogenic epithelium.

Localization of pain-related brain activation: a meta-analysis of neuroimaging data.

A meta-analysis of 140 neuroimaging studies was performed using the activation-likelihood-estimate (ALE) method to explore the location and extent of activation in the brain in response to noxious stimuli in healthy volunteers. The first analysis involved the creation of a likelihood map illustrating brain activation common across studies using noxious stimuli. The left thalamus, right anterior cingulate cortex (ACC), bilateral anterior insulae, and left dorsal posterior insula had the highest likelihood of being activated.
