This report presents the case of a 62-year-old male with a history of chronic alcohol abuse who developed deep vein thrombosis (DVT) complicated by extensive bilateral pulmonary embolism (PE) and a right atrial thrombus. The presence of a right atrial thrombus in conjunction with PE and DVT is a rare and serious clinical presentation, often associated with a high thrombotic burden and increased risk of mortality. The patient initially presented with worsening shortness of breath following an occupational injury that resulted in a left heel laceration, subsequently leading to a significant thromboembolic event.

Objectives: To compare the need for mechanical cervical dilatation following vaginal misoprostol or synthetic osmotic dilator (Dilapan-S) usage for cervical preparation before operative hysteroscopy.

Materials And Methods: Fifty-five premenopausal women scheduled for operative hysteroscopic procedures with a 26 Fr resectoscope were included in this randomized, controlled clinical trial. After randomization, either 400 μg of vaginal misoprostol or intracervical synthetic osmotic dilator (Dilapan-S) was inserted 12 h before operative hysteroscopy.

In decompensated cirrhosis, massive ascites and pleural effusion (hepatic hydrothorax) can be complicated by infection, which manifests either as spontaneous bacterial peritonitis (SBP) or spontaneous bacterial empyema (SBE). SBE is a distinct and often underdiagnosed complication having different pathogenesis and treatment strategy when compared with parapneumonic empyema. Hepatic hydrothorax in the absence of ascites is rare in patients with cirrhosis.

  • Arterial and venous thrombosis, a serious complication of COVID-19, was studied by analyzing D-dimer levels in hospitalized patients from May to September 2020.
  • A total of 720 patients were classified into three groups based on their D-dimer levels, and although 8 thrombotic events occurred, no significant differences were found in D-dimer levels between those who developed thrombosis and those who did not.
  • The study concluded that while a six-fold increase in D-dimer was linked to a higher risk of death, it wasn't a reliable predictor of thrombosis, suggesting that anticoagulation treatment shouldn't solely rely on D-dimer levels.

Increase in the incidence of drug resistance and association with HIV has led to a resurgence of tuberculosis. However, tubercular arteritis continues to remain a rare entity with a prelidection for the thoracic aorta. We report a tubercular ascending aortic pseudoaneurysm in a patient already on treatment for disseminated tuberculosis who underwent successful surgical repair and also review literature pertaining to this entity.
