Introduction: The MERIT study in Malaysia is a real-world retrospective, observational, multicenter study that evaluated asthma control in patients with uncontrolled asthma who were switched from as-needed (pro re nata [PRN]) budesonide/formoterol or inhaled corticosteroid (ICS) whenever a short-acting beta-agonist (SABA) was taken, to proactive regular dosing of fluticasone propionate/salmeterol (FP/SAL PRD).

Methods: Data from the medical records of patients who were stepped up to FP/SAL PRD were extracted retrospectively at baseline and follow-up (between 3 and 6 months after stepping up to FP/SAL PRD). The primary endpoint was the percentage of patients with improvement in asthma control assessed via the Asthma Control Test (ACT).

Introduction: Loss to follow-up (LTFU) and smoking during TB treatment are major challenges for TB control programs. Smoking increases the severity and prolongs TB treatment duration, which lead to a higher rate of LTFU. We aim to develop a prognostic scoring tool to predict LTFU among TB patients who smoke to improve successful TB treatment outcomes.

The diagnosis of endobronchial tuberculosis (EBTB) is difficult as it is not well visualized radiologically, and bronchoscopy is not routinely performed for tuberculosis (TB) patients. Bronchoscopic characterization via endoscopic macroscopic features can speed up the diagnosis of EBTB and prompt immediate treatment. In this study, we identified the clinical and bronchoscopic morphology of 17 patients who were diagnosed with EBTB from 2018 to 2020.

Article Synopsis
  • Dysglycemia is common in patients with chronic obstructive pulmonary disease (COPD) but is often undiagnosed, leading to a study to assess its prevalence and related factors among these patients.
  • The study included 186 adults with established COPD and found that 16 had diabetes and 32 had prediabetes after an oral glucose tolerance test, with higher incidences of newly diagnosed dysglycemia.
  • Key predictors for dysglycemia were identified as cumulative hospital admission days (≥6 days/year) and weight (≥65 kg), highlighting the need for better screening in COPD patients, especially those with frequent acute exacerbations.

Background: Available data on influenza burden across Southeast Asia are largely limited to pediatric populations, with inconsistent findings.

Methods: We conducted a multicenter, hospital-based active surveillance study of adults in Malaysia with community-acquired pneumonia (CAP), acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and acute exacerbation of asthma (AEBA), who had influenza-like illness ≤10 days before hospitalization. We estimated the rate of laboratory-confirmed influenza and associated complications over 13 months (July 2018-August 2019) and described the distribution of causative influenza strains.

The elusive goal of asthma management guidelines is to achieve and maintain good asthma control in asthmatic patients. Against a background of long-term respiratory limitations when living with asthma, stigma and low self-esteem have also been identified as the social phenomenon among adult asthmatics. This study aimed to assess the levels of self-stigma, self-esteem, and asthma control, and to investigate the impact of self-stigma and self-esteem as psychosocial factors on asthma control in Malaysian adults living with asthma.

A method based on extended cubic B-spline is proposed to solve a linear system of second-order boundary value problems. In this method, two free parameters, [Formula: see text] and [Formula: see text], play an important role in producing accurate results. Optimization of these parameters are carried out and the truncation error is calculated.

Article Synopsis
  • - Teratomas in the anterior mediastinum are uncommon, often grow slowly without symptoms, and are usually found incidentally through chest imaging.
  • - Mycobacterium abscessus is a type of bacteria that can cause infections in humans, often following skin injuries or surgery, and is relevant in cases with unusual post-operative complications.
  • - A case is presented where a benign cystic teratoma was mistaken for pleural effusion, leading to an M. abscessus infection after thoracotomy; this highlights the need for thorough evaluation when dealing with unusual diagnoses and persistent infections.

Background And Objective: The stigmatisation degree, self-esteem and knowledge either directly or indirectly influence the control and self-management of asthma. To date, there is no valid and reliable instrument that can assess these key issues collectively. The main aim of this study was to test the reliability and validity of the newly devised and translated "Stigmatisation Degree, Self-Esteem and Knowledge Questionnaire" among adult asthma patients using the Rasch measurement model.

We report a case of a 34-year-old lady with past history of asthma and pulmonary tuberculosis, who presented 5 weeks pregnant with acute dyspnea. Her chest X-ray showed left-sided complete lung collapse and concomitant right-sided pneumothorax. The pneumothorax was initially managed conservatively with a chest tube but due to its persistence despite suction, was subsequently changed to a Pneumostat(TM), with which she was later discharged.

The generalized nonlinear Klien-Gordon equation plays an important role in quantum mechanics. In this paper, a new three-time level implicit approach based on cubic trigonometric B-spline is presented for the approximate solution of this equation with Dirichlet boundary conditions. The usual finite difference approach is used to discretize the time derivative while cubic trigonometric B-spline is applied as an interpolating function in the space dimension.
