Photoletter to the editor: Subcutaneous ciliated Mullerian cyst.

Cutaneous ciliated cysts are benign lesions occurring primarily on the lower extremity of girls and young women. We present a case of a cutaneous ciliated Mullerian cyst arising in the lower leg of a 14-year-old girl, with brief discussion of etiology and diagnosis. This is a rare entity with approximately 50 cases in the literature.

Renal-adrenal fusion: instance of an adrenal adenoma mimicking a solid renal mass at CT--case report.

The authors report an unusual case of unilateral renal-adrenal fusion with a concurrent adrenal adenoma. At computed tomography, this abnormality appeared as a solid enhancing lesion in the upper pole of the kidney, mimicking a renal mass. The ambiguous characteristics of this lesion at cross-sectional imaging, along with alternative diagnostic possibilities, are discussed.

Correlates of HIV infection among incarcerated women: implications for improving detection of HIV infection.

The prevalence of HIV infection in correctional settings is several-fold higher than found in community settings. New approaches to identifying HIV infection among prisoners are urgently needed. In order to determine the HIV seroprevalence and to identify the correlates of HIV infection among female prisoners, an anonymous, but linked HIV serosurvey was conducted at Connecticut's sole correctional facility for women (census=1,100).

Nonurgent emergency department patient characteristics and barriers to primary care.

Objective: Nonurgent (NU) emergency department (ED) use is at the forefront of medico-political agendas, and diversion of NU patients has been entertained as a management strategy. Before policy changes are implemented, this population should be better understood with respect to their characteristics and reasons for not presenting to primary care providers (PCPs) instead of EDs. This study compares NU with urgent and semiurgent (USU) patients and describes the NU patients' reasons for not seeking care with a PCP before presenting to the ED.

Developing a directly administered antiretroviral therapy intervention for HIV-infected drug users: implications for program replication.

Directly administered antiretroviral therapy (DAART) is one approach to improving adherence to among human immunodeficiency virus (HIV)-infected drug users. We evaluated the essential features of a community-based DAART intervention in a randomized, controlled trial of DAART versus self-administered therapy. Of the initial 72 subjects, 78% were racial minorities, and 32% were women.

Impact of ambulance transportation on resource use in the emergency department.

Objective: To determine how ambulance transportation is associated with resource use in the emergency department (ED).

Methods: A retrospective administrative database review of patient visits to a Montreal tertiary care hospital ED in one year (April 2000-March 2001). Measures of resource use included ED length of stay, admission to the hospital, and whether consultations and radiology/imaging tests (excluding plain-film x-rays) were ordered from the ED.
