As a potential corridor connecting Southwest Asia with western and northern Europe, the Armenian Highlands and southern Caucasus hold great potential for increasing our understanding of Upper Paleolithic behavioral and cultural variability. However, given the dearth of Upper Paleolithic sites, we lack the data necessary to answer basic questions regarding the timing and nature of the Upper Paleolithic in this region. Solak-1 is an open-air site located along the upper Hrazdan Valley (1635 m above sea level) in central Armenia.

Background: Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is a common surgical procedure for ulcerative colitis and familial adenomatous polyposis. IPAA strictures are a known complication, often requiring surgical intervention. Endoscopic interventions offer a less invasive alternative, but their safety and efficacy remain uncertain.

Background: The risk of gastrointestinal (GI) cancer after lung transplantation (LTx) in sarcoidosis patients is not well defined. Given the cancer risks linked to sarcoidosis and organ transplantation, this study investigated the incidence of GI malignancies (DNM), comparing LTx recipients with sarcoidosis or idiopathic pulmonary fibrosis (IPF).

Methods: We analyzed data from the United Network for Organ Sharing registry, including adults with sarcoidosis or IPF who underwent LTx between May 2005 and December 2018.

Background: Enrolling heart failure (HF) patients in clinical studies is challenging. Emergency department (ED) may use students as research associates programs, such as the University of Rochester Medical Center (URMC) ED Research Associate (EDRA) program, to screen and consent patients for clinical studies. This manuscript examines the effectiveness of the URMC EDRA program in consenting HF patients into a clinical study.

Background And Aims: Interest in cold endoscopic mucosal resection (EMR) for colorectal polyps has been growing lately. We conducted a meta-analysis of RCTs to compare cold and hot EMR for colorectal polyps.

Methods: We reviewed several databases from inception to October 06, 2024.

Primary sclerosing cholangitis (PSC) is a risk factor for cholangiocarcinoma. When a child is diagnosed with both PSC and inflammatory bowel disease (IBD), evidence-based information on counseling families and risk management of developing cholangiocarcinoma is limited. In this case series (PubMed/collaborators), we included patients with PSC-IBD who developed cholangiocarcinoma and contacted authors to determine an event curve specifying the time between the second diagnosis (IBD or PSC) and a diagnosis of cholangiocarcinoma.

Background: IMPLEMENT-HF demonstrated a virtual team-based care strategy was safe and improved prescription of guideline directed medical therapy (GDMT) in hospitalized patients with heart failure and reduced ejection fraction (HFrEF). We evaluated differences in efficacy and safety outcomes by ethnicity in IMPLEMENT-HF.

Methods: IMPLEMENT-HF evaluated a provider-facing virtual team-based care strategy vs.

Background: Per-oral endoscopic myotomy (POEM) is an effective option for treating achalasia. Despite its high efficacy, a fraction of patients remain symptomatic after POEM, and the data on the optimal management of these patients is limited. Few studies have evaluated the effectiveness of repeat POEM after a failed POEM.

Researchers in ubiquitous computing have long promised that passive sensing will revolutionize mental health measurement by detecting individuals in a population experiencing a mental health disorder or specific symptoms. Recent work suggests that detection tools do not generalize well when trained and tested in more heterogeneous samples. In this work, we contribute a narrative review and findings from two studies with 41 mental health clinicians to understand these generalization challenges.

Introduction: Endoscopic ultrasound (EUS)-guided drainage of pancreatic fluid collections (PFCs) has variable outcomes according to the type of lumen-apposing metal stent (LAMS) used. We aimed to perform a systematic review and meta-analysis of the available data.

Methods: Online databases, including EMBASE, PubMed, and SCOPUS, were searched from inception to 30 January 2024.

Background: Balloon-assisted enteroscopy (BAE) (both single- and double-balloon enteroscopy) has garnered attention in the treatment of small intestine strictures in patients with Crohn's disease (CD). This study aimed to evaluate the pooled clinical outcomes of BAE-mediated endoscopic dilation of small intestine strictures in patients with CD.

Methods: We searched multiple databases for articles reporting outcomes following BAE for small intestinal strictures in patients with CD.

Article Synopsis
  • The study focuses on developing guidelines for the safe use of fluoroscopy in gastrointestinal endoscopy, balancing its benefits with concerns about radiation exposure to patients and healthcare workers.
  • A modified Delphi method was used, involving three rounds of surveys with 46 experts, resulting in 43 proposed statements, of which 31 achieved consensus and were prioritized across various categories such as Patient Safety and Staff Safety.
  • The final consensus statements highlight the importance of education and safety measures, with a significant majority rated as high priority, aiming to enhance safety culture in healthcare settings while utilizing fluoroscopy.

Mycoplasma pneumoniae pneumonia occurs periodically in an endemic-epidemic pattern. The incidence of this disease has fallen drastically as a result of the health measures imposed for the COVID-19 pandemic. In Switzerland, as elsewhere in the world, a new epidemic has been affecting pediatrics and adult populations since September 2023.

The relaxation behaviour of isolated quantum systems taken out of equilibrium is among the most intriguing questions in many-body physics. Quantum systems out of equilibrium typically relax to thermal equilibrium states by scrambling local information and building up entanglement entropy. However, kinetic constraints in the Hamiltonian can lead to a breakdown of this fundamental paradigm owing to a fragmentation of the underlying Hilbert space into dynamically decoupled subsectors in which thermalization can be strongly suppressed.

  • The study examines the consistency of differentially expressed genes (DEGs) in Alzheimer's Disease (AD), Parkinson's Disease (PD), and COVID-19 by analyzing multiple published datasets.
  • It finds that while DEGs in PD and COVID-19 datasets can reasonably predict case status in other studies, DEGs in AD datasets perform poorly on this front.
  • A new meta-analysis method (SumRank) was developed, enhancing predictive power by integrating DEGs across datasets and resulting in the identification of both known and new biological pathways related to these diseases.

Article Synopsis
  • The study focuses on developing a simple risk score model to predict mortality in ICU patients with acute pancreatitis, aiming to improve treatment planning.
  • Researchers analyzed data from 380 patients, identifying key predictive variables through advanced statistical methods, ultimately creating a model with five key indicators.
  • The final model demonstrated high accuracy in predicting both 7-day and 30-day mortality, outperforming existing prediction scores like BISAP and APACHE-II, thus providing a practical tool for ICU clinicians.

Article Synopsis
  • IgG4 pancreaticobilliary disease (IgG4-PBD) often recurs after glucocorticoid treatment, leading researchers to investigate rituximab (RTX) as a potential solution to prevent relapses.
  • A systematic review and meta-analysis assessed available studies on the efficacy and safety of RTX in treating IgG4-PBD, analyzing outcomes like remission and adverse events.
  • Results showed that RTX led to a 68% rate of complete remission and a low relapse rate of 23%, with few adverse events, indicating RTX is a promising treatment for managing recurrent IgG4-PBD, particularly for patients who can’t tolerate steroids.
