Importance: The existing models predicting right ventricular failure (RVF) after durable left ventricular assist device (LVAD) support might be limited, partly due to lack of external validation, marginal predictive power, and absence of intraoperative characteristics.

Objective: To derive and validate a risk model to predict RVF after LVAD implantation.

Design, Setting, And Participants: This was a hybrid prospective-retrospective multicenter cohort study conducted from April 2008 to July 2019 of patients with advanced heart failure (HF) requiring continuous-flow LVAD.

Article Synopsis
  • Long COVID affects 25-30% of people who have had SARS-CoV-2, leading to symptoms like extreme fatigue, difficulty in performing daily activities, and rapid heart rate.
  • There is a need for specific treatment strategies to help manage these symptoms, particularly through tailored exercise programs that accommodate the unique challenges faced by Long COVID patients.
  • This review emphasizes that structured exercise can counteract the effects of deconditioning, which often results from prolonged inactivity, and may help reduce the symptom burden in those suffering from Long COVID.

Massive pulmonary embolism (PE) is associated with high mortality rates. Pulmonary Embolism Response Team (PERT) collaboration with prompt access to veno-arterial extracorporeal membrane oxygenation (VA ECMO) during mechanical or aspiration thrombectomy for massive PE can be life-saving; ECMO stand-by should be considered for high-risk cases. We describe a case of massive PE treated with intraprocedural VA ECMO during the catheter-directed intervention.

Background: Benign external compression of the Inferior Vena Cava (IVC) with distal thrombus formation is seldomly described in the medical literature.

Case Presentations: We report a case of external IVC compression by a dilated right renal pelvis and hydronephrotic kidney secondary to longstanding Ureteropelvic Junction (UPJ) obstruction found in a 68-year-old male. Management included therapeutic anticoagulation, IVC filter placement, attempted thrombectomy by interventional radiology, and interval repeats imaging.

Fibromuscular dysplasia is a nonatherosclerotic, under-recognized disorder primarily seen in middle-aged women. It can lead to several complications, such as hypertension, headaches, dissections, aneurysms, myocardial infarctions, and cerebrovascular accidents, to name a few. This article provides a comprehensive review of current literature on epidemiology, etiology, diagnosis, treatment, and long-term surveillance and fibromuscular dysplasia management.

Ineffective communication between nursing staff and residents leads to numerous educational and patient-care interruptions, increasing resident stress and overall workload. We developed an innovative and simple, secure electronic health record (EHR) base text paging system to communicate with internal medicine residents. The goal is to avoid unnecessary interruption during patient care or educational activities and reduce stress.
