Background: Limitations to using the knee as donor cartilage include cartilage thickness mismatch and donor site morbidity. Using the radial head as donor autograft for capitellar lesions may allow for local graft harvest without distant donor site morbidity. The purpose of this study is to demonstrate the feasibility of performing local osteochondral autograft transfer from the nonarticular cartilaginous rim of the radial head to the capitellum.

We report a case of cutaneous melioidosis in a 54-year-old male with a meropenem-resistant sub-population. He was empirically treated with episodic doxycycline and trimethoprim-sulfamethoxazole; however, the abscess re-accumulated. The patient had no prior exposure to meropenem.

Droplet microfluidic platforms have been broadly used to facilitate DNA transfer in mammalian and bacterial hosts via methods such as transformation, transfection, and conjugation, as introduced in our previous work. Herein, we recapitulate our method for conjugal DNA transfer between strains in a droplet for increased conjugation efficiency and throughput of an otherwise laborious protocol. By co-incubating the donor and recipient strains in droplets, our method confines cells into close proximity allowing for increased cell-to-cell interactions.

Future climate change is expected to result in tree species shifting their geographic distributions in ways that could reorganize species into assemblages with no contemporary analog. These projected no-analog forests raise concern as their ecological function could similarly shift, which may challenge established conservation and management efforts. Here, we implement a community-level modelling approach to identify the key climatic and topographic drivers of forest composition in North America, and then use these models to predict the distribution of "disappearing" and "novel" forest assemblages in future climate.

Article Synopsis
  • The LightFix Trial aimed to assess the effectiveness of the IlluminOss System (IS) in treating impending and completed pathological fractures of the humerus over one year.
  • A study with 81 patients showed significant decreases in pain and increases in upper limb function after treatment, with notable improvements in pain scores and functional metrics throughout the year.
  • While the IS provided benefits, it had a device fracture rate of 15%, suggesting caution when used alone for completed fractures.

Chronic kidney disease (CKD) is the progressive loss of kidney function/structure over a period of at least 3 months. It is characterised histologically by the triad of cell loss, inflammation and fibrosis. This literature review focuses on the forms of cell death that trigger downstream inflammation and fibrosis, collectively called regulated cell death (RCD) pathways.

  • Cancer can lead to serious kidney issues in children requiring kidney replacement therapy (KRT), with a study identifying 287 pediatric KRT patients who had a history of cancer.
  • The study showed that those whose cancer caused KRT had a longer wait for kidney transplantation compared to matched controls, with a median time of 2.4 years for cancer patients versus shorter times for controls.
  • While childhood cancer survivors experienced increased mortality rates while on KRT (16% for group 1 and 23% for group 2) compared to controls, their long-term survival rates after kidney transplantation were similar to non-cancer patients.

Despite a significant increase in utilization over the past decade, the number of donation after circulatory death (DCD) organs that are procured and transplanted in the United States (US) remains well below its potential. There is still room for expansion, as utilizing DCD organs to the fullest extent is currently the most viable solution to the persistent mismatch between supply and demand in transplantation. We convened a multidisciplinary transplantation summit to examine various aspects of DCD, with faculty members from around the world with clinical and academic interest in DCD donation and transplantation, including abdominal and cardiothoracic surgeons, organ procurement organization directors, hepatologists, and gastroenterologists.

Ischaemic spinal cord injury remains a significant challenge in thoracoabdominal aortic repairs. Modern techniques have reduced spinal cord injury rates yet managing patients during and after thoracoabdominal aortic repairs remains complex. This article outlines our comprehensive approach to the prevention of spinal cord injuries in open thoracoabdominal aortic repair operations, focusing on the placement of cerebrospinal fluid drain and intraoperative strategies to enhance spinal cord protection.

Article Synopsis
  • Biologics and small-molecule therapies have improved the treatment landscape for atopic dermatitis, but older systemic treatments are still in use.
  • The PEDIatric STudy in Atopic Dermatitis is a 10-year observational study tracking the effectiveness and safety of dupilumab, cyclosporine, and methotrexate in children under 12 with moderate-to-severe atopic dermatitis, with interim results available after 2 years.
  • Results show that dupilumab led to better improvements in skin severity, itching, and fewer discontinuations or adverse effects compared to methotrexate and cyclosporine.

Nursing's efforts at organizing in the United States have encompassed various approaches to creating change at a systemic and political level, namely shared governance, professional associations, and nurse unions. The United States is currently experiencing the effects of an authoritarian sociopolitical agenda that has taken aim at our profession's ethic of providing equitable care for all people through legislation that bans gender-affirming care and abortions. Nursing is simultaneously experiencing a crisis of burnout and moral distress, as we navigate the everyday functions of a for-profit healthcare system under the Capitalocene.

During the 2023-2024 influenza season, the Republic of Moldova, a lower-middle income country seeking accession into the European Union, independently financed their influenza vaccine supply transitioning from external support from the Partnership for International Vaccine Initiatives, a collaboration conceived in 2015. As part of this transition, a mixed-methods evaluation was conducted from May 2023 - January 2024 to identify current strengths and weaknesses of the influenza vaccination program. A total of 157 interviews were conducted: one with the National Immunization Program (NIP), six with district health officers, 18 at health facilities, 18 with caregivers/parents, 34 with healthcare workers, 43 with adults with chronic diseases, 19 with pregnant women, and 13 vaccine observation sessions; further five expert interviews with an international organization, the insurance company, senior government officials in public health and within the ministry of health, and those involved with COVID-19 were conducted.

Biosecurity measures preventing exposure of pigs to rodents, wildlife, and contaminated feed or waste products reduce the risk of zoonotic infection in pork. To understand the benefits of such measures in the United States, we conducted the first comprehensive survey of pigs produced under the Pork Quality Assurance Plus production standard, surveying 3,208,643 pork samples from twelve processing locations tested over a period of 54 months. We detected no sp.

Gastric malabsorptive conditions may prevent patients from deriving benefit from orally administered medications intended for enteric absorption. While malabsorption is an increasingly common issue, current data on alternative oral options for agitation in these patients are very sparse. Sublingual (SL) asenapine is absorbed transmucosally, bypassing gut absorption, making it a viable consideration.

The wetsalts tiger beetle, Cicindelidia haemorrhagica (LeConte) (Coleoptera: Cicindelidae), is found in several active thermal hot spring areas in Yellowstone National Park (YNP) where substrate surface temperatures can exceed 50 °C. However, relationships between surface temperatures and the time adults spend on them remain poorly understood. Therefore, we characterized thermal profiles of Dragon Spring and Rabbit Creek, 2 thermally active research sites containing C.
