Total colectomy with ileostomy placement is a treatment for patients with inflammatory bowel disease or familial adenomatous polyposis (FAP). A rare and late complication of this treatment is carcinoma arising at the ileostomy site. We describe two such cases: a 78-year-old male 30 years after subtotal colectomy and ileostomy for FAP, and an 85-year-old male 50 years after colectomy and ileostomy for ulcerative colitis.

Substrates both specific and unspecific for pancreatic enzymes abolish and modify the inhibition of pancreatic enzymes secretion induced by their intraduodenal administration. Enterosorbents and gastric mucus, depending on the strength of their sorption properties, abolish or decrease the inhibition of secretion of pancreatic enzymes induced by intraduodenal administration of pancreatic enzymes. Three series of acute experiments were carried out on dogs. In the first series pancreatic enzyme secretion (PES) was stimulated by intravenous injection of secretin and pancreozymin and selectively inhibited by intraduodenal injection of amylase; in the second series PES was induced by intraduodenal injection of acidified hydrolysin and nonselectively inhibited by intraduodenal injection of PES from the same dog; in the third series of experiments PES was stimulated by intraduodenal injection of hydrolysin, and selectively inhibited by intraduodenal injection of amylase. Intraduodenal administration of pancreatic secret into the dog intestine inhibited the secretion of enzymes by the pancreas. This inhibitory effect is eliminated or diminished by simultaneous administration of the enzyme adsorbents: activated carbon and gastric mucus. Self-regulatory effects of solubilized and absorbed enzymes in duodenal chyme seem to differ from each other.
