Objective: To develop individualized approaches to the use of neuromodulation as a non-pharmacological treatment of cognitive impairment (CI) based on the assessment of compensatory brain reserves in functional MRI (fMRI).

Material And Methods: Twenty-one adults over 45 years of age, representing a continuum from healthy norm to mild cognitive impairment due to aging and early cerebral small vessel disease, were studied. Acute disseminated encephalomyelitis (AEM) is an immune-mediated inflammatory demyelinating disease of the central nervous system (CNS), usually single-phase. In WREM, multiple lesions of the central nervous system of an inflammatory-demyelinating nature accompanied by extremely polymorphic neurological disorders develop acutely or subacutely. The review considers the issues of epidemiology, trigger factors of the inflammatory process, clinical manifestations, differential and molecular diagnostics of WECM, and outlines the ways for further study of this pathology.

Long-term outcomes of surgery have been analysed for 684 patients with OMCP in terms of possible implications of the severity and kind of pathological processes in the middle ear as well as the operative procedure on functional and anatomic results of tympanoplasty. Early surgical interventions in OMCP are preferable because they allow "closed" tympanoplasty. Maximal removal of hypertrophic inflammation-affected parts of the tympanic mucosa promotes functional effectiveness of the operation in view of higher position of the stapes achieved with the help of combined prostheses.
