Eighty patients with ischemic heart disease and stable angina pectoris of effort were examined for the efficacy of dinitrosorbilong (DNL), a new long-acting drug based on isosorbide dinitrate (ID). The anti-ischemic effect of DNL appraised with the aid of repeated exercise lasted about 8h, which was on the average 4 h longer as compared with the duration of the anti-ischemic effect of ID tablets. The regular intake of DNL favoured an appreciable decrease in the number of angina pectoris attacks making it possible to lower nitroglycerin consumption.

Using exercise tests in 22 patients with stable angina of effort, the authors studied the possibility of the development of tolerance to the antianginal effect of isosorbide dinitrate (ID) and verapamil (VP). Within 6-12 weeks of the regular intake of ID, 5 (26%) patients developed complete tolerance, 6 (32%) partial tolerance to the drug. In 8 (42%) patients the effect of ID remained stable.

Fifteen patients with coronary heart disease and stable angina pectoris of effort were examined for tolerance to the antianginal effect of isosorbide dinitrate (ID) taken permanently. After 6 weeks of treatment with ID in doses selected on an individual basis the antianginal effect evaluated by bicycle ergometry before and after ID intake remained stable. Sixty per cent of the patients showed a decrease in the drug effect, while in 40 per cent of the patients that decrease was moderate (partial ID tolerance) and in 20 per cent considerable (marked ID tolerance).
