On the case of a patient who had medical care on account of a febrile disease diagnosed as pleuritis the authors draw attention to a relatively new pathological unit described as the "Brugada syndrome". It is a genetically conditioned anomaly of sodium channels, which, when fully expressed, is manifested by a picture resembling right bundle branch block, elevation of ST segment in leads V1-V3 and the occurrence of malignant ventricular arrhythmias or sudden death without structural heart disease. Neither the reported patient nor his family suffered from arrhythmias.

The authors investigated the incidence of cardiac complications in patients after elective pelvic reconstructions on account of ischaemic disease of the lower extremities in 1985-1990 and in 1990-1993. The first group of patients was examined by an experienced internist and the second by a cardiologist. The second group had a substantially higher percentage of risk factors than the first one.

The authors tested the importance of the load score for assessment of the prognosis of patients after myocardial infarction and for assessment of the severity of the atherosclerotic affection of the coronary arteries. In 1984-88 the authors examined in 813 subjects with the chest pain syndrome of varying aetiology (acute myocardial infarction, myocarditis, pericarditis, vertebrogenic algic syndrome, embolism of the pulmonary artery, patients lacking detectable organic causes of pain) the trend of myoglobin serum levels. They found significantly elevated values only in patients with myocardial infarction and myocarditis whereby the two diseases differ in particular as regards the shape of the curve of myoglobin values. In chest pain with another aetiology the myoglobin levels rose only rarely or not at all.
