Article Synopsis
  • The French National Authority for Health (HAS) has released new guidelines for patient blood management (PBM) during surgeries, focusing on three key areas: optimizing red blood cell count, minimizing blood loss, and managing anemia tolerance.
  • Preoperative recommendations include identifying anemia and iron deficiency early, especially for surgeries with a higher risk of bleeding.
  • Intraoperative strategies involve using tranexamic acid and certain surgical techniques to control bleeding, while postoperative guidance emphasizes reducing blood sampling and monitoring for anemia to determine if treatments like intravenous iron are needed.

The objective was to establish new diagnostic criteria for undernutrition for the French population, concordant for children aged <18 years and adults aged <70 years, easy to use by health professionals and applicable whatever the situation (in and outpatients). A multi-disciplinary working and a reading group were involved. The procedure was divided into four phases: (1) systematic review and synthesis of the literature; (2) writing of the initial version of the guidelines; (3) reading and (4) finalisation.

Sixteen new isothiazoloisoxazole 1,1-dioxides, one new isothiazolotriazole and one new isothiazolopyrazole have been synthesised by using 1,3-dipolar cycloadditions to isothiazole 1,1-dioxides. One sub-set of these isothiazoloisoxazoles showed low μM activity against a human breast carcinoma cell line, whilst a second sub-set plus the isothiazolotriazole demonstrated an interesting restricted rotation of sterically hindered bridgehead substituents. A thiazete 1,1-dioxide produced from one of the isothiazole 1,1-dioxides underwent conversion into an unknown 1,2,3-oxathiazolin-2-oxide upon treatment with Lewis acids, but was inert towards 1,3-dipoles and cyclopropenones.
