The authors have studied the findings of the examination of and therapeutic outcomes in a total of one hundred and thirty-one patients diagnosed with atherosclerotic-aetiology stage IV chronic arterial insufficiency of the lower extremities. Of these, the dynamic analysis of the tissue blood flow on the background of haemodynamically efficient direct revascularization of the lower limbs was carried out in ninety-six patients. In a further 35 patients, a comprehensive instrumental examination of the blood flow on the background of chronic critical ischaemia was complemented by evaluating the morphological alterations in the skin of the foot of the affected extremity.

One of modern and highly effective methods of diagnostics and differential diagnostics of infected pancreonecrosis is chromatographic technique, which allows for identification of anaerobic non-clostridial infection in the foci of pancreatic destruction by the presence of volatile fatty acids, the specific end products of anaerobic bacterial metabolism. Gas chromatography (GC) - mass spectrometry (MS) also make it possible to detect in peripheral blood of patients with pancreatitis certain metabolites (di-, polyamines, and aromatic amines) that are markers of tissue (protein structure) disintegration and the degree of pancreonecrosis, which is a valuable indicator of the degree of pancreonecrosis. Results of vascular reconstructions with the following sanifying procedures performed in 82 patients with stage IV chronic arterial lower limb insufficiency are analyzed. Based on complex evaluation of microcirculation three types of tissues ischemic lesion are revealed: irreversible, severe and mild. Patients with irreversible lesion of the tissues of a distal part of the lower extremity require vascular reconstruction with simultaneous amputation at the upper third of the shank.

Videoendoscopic technologies were used in the treatment of postoperative complications in 289 patients. All repeated laparoscopic procedures in early postoperative period after laparoscopic and laparotomic surgeries are integrated under the term "relaparoscopy". The algorithm of choice of a surgical method during laparoscopic examination is presented, technical principles of laparoscopic surgeries, indications and contraindications are formulated.
