Study protocol: multi-centre, randomised controlled clinical trial exploring stromal targeting in locally advanced pancreatic cancer; STARPAC2.

Background: Pancreatic cancer (PDAC: pancreatic ductal adenocarcinoma, the commonest form), a lethal disease, is best treated with surgical excision but is feasible in less than a fifth of patients. Around a third of patients presentlocally advanced, inoperable, non-metastatic (laPDAC), whose stadrd of care is palliative chemotherapy; a small minority are down-sized sufficiently to enable surgical excision. We propose a phase II clinical trial to test whether a combination of standard chemotherapy (gemcitabine & nab-Paclitaxel: GEM-NABP) and repurposing All Trans Retinoic Acid (ATRA) to target the stroma may extend progression-free survival and enable successful surgical resection for patients with laPDAC, since data from phase IB clinical trial demonstrate safety of GEM-NABP-ATRA combination to patients with advanced PDAC with potential therapeutic benefit.

Sex-Related Differences in Motor Unit Firing Rate and Pennation Angle.

Motor unit firing rate (MUFR) and pennation angle were measured concurrently in males and females from submaximal to maximal intensities. Thirty participants, (16F and 14M) performed isometric dorsiflexion contractions at 20%, 40%, 60%, 80% and 100% of maximal voluntary contraction (MVC). During each contraction, measures of MUFR were obtained via surface electromyography decomposition, and muscle fiber pennation angle and fascicle length were obtained via ultrasound.

Aim: Artificial intelligence (AI) chatbots such as Chat Generative Pretrained Transformer-4 (ChatGPT-4) have made significant strides in generating human-like responses. Trained on an extensive corpus of medical literature, ChatGPT-4 has the potential to augment patient education materials. These chatbots may be beneficial to populations considering a diagnosis of colorectal cancer (CRC).

Mapping hazards to the global food system.

Environmental hazards associated with the global food system threaten societal integrity. Yet, there is a major data gap in the global understanding of how the prevalence of hazards is changing over time, how different classes of hazard are distributed, and whether the combined literature represents hazard prevalence equitably across research, policy and legislation, and news. Here, we explore this data gap, leveraging global research, policy, and news databases.

The Rise of Mpox in a Post-Smallpox World.

Reports of mpox are rising in Africa where the disease is endemic and in new countries where the disease has not been previously seen. The 2022 global outbreak of clade II mpox and an ongoing outbreak of the more lethal clade I mpox highlight the pandemic potential for monkeypox virus. Waning population immunity after the cessation of routine immunization for smallpox plays a key role in the changing epidemiologic patterns of mpox.
